Change Request Form definition

Change Request Form a written request for a Change which is submitted by one party to the other pursuant to the Change Control Procedure and in the form set out in Appendix B. Charges: the fees payable by the Customer to Arrow for the Services including but not limited to the services provided by Arrow and its Approved Suppliers, the Equipment, the Software, the Arrow software, and any Additional Services, as set out in a Change Control Note. Commencement Date: the date this Agreement takes effect and comes into force.
Change Request Form means the Change Request Form provided by Canoe.
Change Request Form means a document in substantially the same form as that in Schedule 5 or such other form approved by the Customer. Claim means any allegation, cause of action, liability, claim, proceeding, suit or demand of any nature, whatsoever arising, and whether present or future, fixed or unascertained, actual or contingent and whether at Law, under statute or otherwise. Commencement Date means the date specified as such in the Order Form.

Change Request Form means the document provided under this Schedule which shall be used to acknowledge the amendments agreed by both Parties of the Change;
