Sentinel Events definition

Sentinel Events. Means an “Event” or "unexpected occurrence” involving death or serious Physical or psychological injury, or the risk thereof. Serious injury specifically includes loss of limb or function. The phrase, “or the risk thereof” includes any process variation for which a recurrence would carry a significant chance of a serious adverse outcome."
Sentinel Events means an unexpected Incident, related to system or process deficiencies and/or human error, which leads to death or Major and Enduring Loss of Function for a recipient of healthcare services.
Sentinel Events shall have the meaning set forth in Section 3.10 hereof.

Event: Means any and all of the following; relocation of a consumer due to licensing issues; relocation of the service site or administrative operations of the PROVIDER for more than 24 hours; conviction of a PROVIDER staff for any offense related to the performance of their job duties/responsibilities; Unusual incidents such as emergency medical treatment, hospitalization, medication error, arrest of a consumer, behavioral incidents that are unexpected/not addressed, harm to self, and harm to others. An Event must be in writing within 24 hours and is generally reported to the PAYOR on an “Incident Report”. Service area means the county(ies) served by the PAYOR. Sentinel Events. Provider agrees to review incidents as identified by MSHN and MDHHS. Within 3 business days of the critical incident the Proivder will determine if the event is sentinel. If the critical incident is determined to be sentinel, a root cause analysis (RCA) is required to commence within 2 business days of the determination of the sentinel event. Sentinel events and critical incidents are to be reported as indicated in the reporting requirements.
