Provider shall report any Critical Incidents and Sentinel Events involving Covered Persons immediately upon receipt to CMHSP’s CEO or the CEO’s designated representative and, as appropriate, to MDHHS, the applicable licensing agency, or other agency of the State of Michigan (Adult or Children’s Protective Services), law enforcement, or other public agency, as required by law.

Service Provider shall ensure that it and all Subcontractors have in place policies and procedures for immediate response and reporting of relevant information about Sentinel Events (as defined by MDHHS) to the Authority and that they shall promptly and accurately report any Sentinel Event to the Authority in accordance with applicable state and Authority policies and guidelines.
Sentinel Events are to be reported to MDHHS via MSHN within 24 hours of the event or notification of the death.
