Account Enrollment. In order to become a member of the Developer Program you must register for an account. Your registration must include the name of any company that employs You or that You represent and on whose behalf You are entering into this Agreement. You must select a username and password as part of the security procedures, you must treat such information as confidential, and you must not disclose it to any other person or entity. You also acknowledge that your account is personal to you and agree not to provide any other person with access to the Site or portions of it using your user name, password, or other security information. You agree to notify Us immediately of any unauthorized access to or use of your user name or password or any other breach of security. We have the right to disable any user name, password, or other identifier at any time in Our sole discretion for any or no reason, including if in Our opinion, You have violated any provision of this Agreement
Account Enrollment. STI Participant shall establish an account with the Policy Administrator through the STI Participant Portal. Enrollment requires and constitutes acceptance of this Agreement, as it may change from time to time in accordance with its terms. The person that submits the account registration represents and warrants that he or she has the legal authority to bind the STI Participant to this Agreement.

You must restrict access to Your Developer Account solely to Authorized Users. Interswitch may require You to change Your Developer Account password from time to time at its sole discretion. If You do not change your password within the respective time frame provided by Interswitch, You may get locked out of Your Developer Account.

Account Enrollment. 3.1 Supplier shall work with the relevant LDC to enroll Accounts listed on Transaction Confirmation in a timely manner within the Purchase Period. The Purchase Period shall commence on or about the date set forth under “Start Date”, and end on or about the date set forth under “End Date” in Transaction Confirmation in accordance with the terms of this Agreement. The actual Start Date is dependent on the LDC successfully enrolling the Account(s) and furnishing Supplier with all necessary information regarding the Account(s) meter read cycle and meter read date(s). The dates set forth in the Transaction Confirmation reflect LDC information available at that time or as otherwise estimated by Supplier. The actual meter read dates may occur on or about the dates set forth in the Transaction Confirmation. Supplier will use commercially reasonable efforts to begin service to each Account(s) on the actual meter read date on or about the Start Date set forth in Transaction Confirmation. If Supplier is unable to timely enroll an Account, the Start Date will commence on the next regularly scheduled LDC meter read cycle date following successful enrollment. The End Date will remain the same unless extended for a holdover term pursuant to Section 2.1. Suppler shall not be liable for any failure to enroll or drop an Account by the Start and End Date due to circumstances reasonably beyond its control. If Supplier fails to timely effect an enrollment after a grace period of two billing cycles consistent with this Agreement, the Parties shall implement a true-up reimbursing Participant for any increase in costs associated with a positive difference between the rate for such Basic Service actually paid by Participant and the relevant Contract Price each as in effect during the affected period; provided, however, a true-up will not be owed in cases where such delay is attributable to circumstances reasonably beyond Suppliers control such as LDC error or inaccuracy of Participant provided information, not corrected in time to meet enrollment schedules herein, notwithstanding a good faith effort by Supplier to meet such deadlines. 3.2 Supplier shall, subject to Section 5.2, timely take such actions within its reasonable control, including without limitation filing a complete and accurate drop transaction notice with the relevant LDC, necessary and convenient for Participant to receive Basic Service with respect to an Account during any period in which no Contract Price is in e...
Account Enrollment. To access the Service, you must first enroll to establish an individual user account (“Account”), by providing certain information. With the exception of subaccounts established for minor children of whom you are a parent or legal guardian, you agree that you will not create more than one Account, or create an account for anyone other than yourself without first receiving permission from the other person. In exchange for your use of the Service and, if applicable, in order for providers to send notices to you, you agree to: (i) provide true, accurate, current and complete information about yourself as prompted by our Account enrollment form; and (ii) each time you log on, maintain and promptly update such Account information to keep it true, accurate, current and complete. If you provide any information that is untrue, inaccurate, not current or incomplete, or we have reasonable grounds to suspect that such Account information is untrue, inaccurate, not current or incomplete, we reserve the right to suspend or terminate your Account and refuse any and all current or future use of the Service. You represent and warrant that you are at least 18 years of age (19 years of age in Nebraska) and possess the legal right and ability, on behalf of yourself or a minor child of whom you are a parent or legal guardian, to agree to these Terms of Use.

KFHPWA or the Group may require confirmation that when initially offered coverage such persons submitted a written statement declining because of other coverage. Application for coverage must be made within 31 days of the termination of previous coverage. b. KFHPWA will allow special enrollment for individuals who are eligible to be a Subscriber and their Dependents (other than for nonpayment or fraud) in the event one of the following occurs: 1) Divorce or Legal Separation. Application for coverage must be made within 60 days of the divorce/separation. 2) Cessation of Dependent status (reaches maximum age). Application for coverage must be made within 30 days of the cessation of Dependent status. 3) Death of an employee under whose coverage they were a Dependent. Application for coverage must be made within 30 days of the death of an employee. 4) Termination or reduction in the number of hours worked. Application for coverage must be made within 30 days of the termination or reduction in number of hours worked. 5) Leaving the service area of a former plan. Application for coverage must be made within 30 days of leaving the service area of a former plan. 6) Discontinuation of a former plan. Application for coverage must be made within 30 days of the discontinuation of a former plan. c. KFHPWA will allow special enrollment for individuals who are eligible to be a Subscriber and their Dependents in the event one of the following occurs: 1) Marriage. Application for coverage must be made within 31 days of the date of marriage. 2) Birth. Application for coverage for the Subscriber and Dependents other than the newborn child must be made within 60 days of the date of birth. 3) Adoption or placement for adoption. Application for coverage for the Subscriber and Dependents other than the adopted child must be made within 60 days of the adoption or placement for adoption. 4) Eligibility for premium assistance from Medicaid or a state Children’s Health Insurance Program (CHIP), provided such person is otherwise eligible for coverage under this EOC. The request for special enrollment must be made within 60 days of eligibility for such premium assistance. 5) Coverage under a Medicaid or CHIP plan is terminated as a result of loss of eligibility for such coverage. Application for coverage must be made within 60 days of the date of termination under Medicaid or CHIP. 6) Applicable federal or state law or regulation otherwise provides for special enrollment.

  • Enrollment Process The Department may, at any time, revise the enrollment procedures. The Department shall advise the Contractor of the anticipated changes in advance whenever possible. The Contractor shall have the opportunity to make comments and provide input on the changes. The Contractor shall be bound by the changes in enrollment procedures.
