Activities Covered. The scope of the register covers all activities, other than those excluded in part IV, carried out with the objective of directly or indirectly influencing the formulation or implementation of policy and the decision-making processes of the EU institutions, irrespective of the channel or medium of communication used, for example outsourcing, media, contracts with professional intermediaries, think-tanks, platforms, forums, campaigns and grassroots initiatives. These activities include, inter alia, contacting Members, officials or other staff of the EU institutions, preparing, circulating and communicating letters, information material or discussion papers and position papers, and organising events, meetings or promotional activities and social events or conferences, invitations to which have been sent to Members, officials or other staff of the EU institutions. Voluntary contributions and participation in formal consultations on envisaged EU legislative or other legal acts and other open consultations are also included.

Activities Covered a. An Association staff representative; b. A member of the Executive Committee c. A member of the Representative Assembly; d. A member of a committee or task force of the Association, the BCTF, the CTF, or the Teacher College Council; or e. An official representative or delegate of the Association or the BCTF; shall be entitled to release time from instructional duties without loss of pay to carry out the duties involved. Such release from duties shall be granted in accordance with Article A.23.2, subject to the Board being reimbursed for the cost of the teacher on call.

Activities Covered. The scope of the register covers all activities, other than those referred to in paragraphs 10 to 12, carried out with the objective of directly or indirectly influencing the formulation or implementation of policy and the decision-making processes of the EU institutions, irrespective of where they are undertaken and of the channel or medium of communi­ cation used, for example via outsourcing, media, contracts with professional intermediaries, think tanks, platforms, forums, campaigns and grassroots initiatives. For the purpose of this agreement, ‘directly influencing’ means influencing by way of a direct contact or communication with the EU institutions or other action following up on such activities and ‘indirectly influencing’ means influencing through the use of intermediate vectors such as media, public opinion, conferences or social events, targeting the EU institutions. In particular, those activities include: — contacting Members and their assistants, officials or other staff of the EU institutions; — preparing, circulating and communicating letters, information material or discussion papers and position papers; — organising events, meetings, promotional activities, conferences or social events, invitations to which have been sent to Members and their assistants, officials or other staff of the EU institutions; and — voluntary contributions and participation in formal consultations or hearings on envisaged EU legislative or other legal acts and other open consultations.

Activities Covered. This Voluntary Agreement to Release Rights and Waive Liability (“Agreement”) is made in contemplation of any and all activities pursued while I am physically present on the real property of LMC or participating in any activities while an employee, agent, volunteer, guest or invitee of LMC, including but not limited to, any and all aquatic activities such as swimming, diving, scuba diving, snorkeling, snubba diving, kayaking, boating, surfing, body surfing, kite boarding, paddle boarding, stand up paddle boarding, tow-in surfing, long boarding, skim boarding, body boarding, water skiing, wake boarding, hydro foil boarding, wake surfing, wind surfing, canoe paddling, rafting, sailing, rowing, riding wave runners or jet skis, and any and all non-aquatic activities such as driving or riding as a passenger on all-terrain vehicles, beach combing, beach cleanups, turtle walks, turtle watching, tennis, basketball, golf, bocci ball, frisbee, baseball, football/rugby, cricket, ping pong, volleyball, billiards, soccer, net ball, racquet ball, dodge ball, parasailing, hang gliding, paragliding, jet boarding, bungee jumping, go carting, golf cart riding, helicopter riding, submarine riding, sail plane riding, sea plane riding, airplane riding, hot air ballooning, taxi tours, island hopping, skateboarding, hot tubing, swimming pool games, running and jogging, kite flying, fishing, massage, bicycling, weight lifting, yoga, spear fishing, exercising, tree climbing, hiking and wild life viewing, and other related activities on and off shore of the property of LMC (hereinafter the “Released Activities”). I further understand that from time to time, I may be offered the opportunity to participate in other activities not listed above. I understand and agree that such activities are also included within the meaning of the term Released Activities.

Activities Covered. The Division will provide the aids and services required by this Agreement during all regularly scheduled activities (including classes) of a student with diabetes that are conducted by the Division. The Division will also provide these aids and services during all optional and extracurricular activities in which a student with diabetes is an actual participant (rather than merely an observer or audience member) that, although not conducted by the Division, receive significant assistance from the Division, provided that each student or the student’s parent or guardian must provide written notice to the school principal or designee, or other appropriate designated Division staff member, of the student’s participation in the activity at least five school days in advance (email notice is acceptable) unless Division staff already have such notice, e.g., if the student regularly participates in the activity and is on a list of participants in the possession of Division staff at least five school days in advance. All diabetes-related aids and services will be provided by Division staff who have received the proper level of training, as described below. If the Division is of the opinion that a particular activity that is sponsored by the Division or takes place on Division property does not receive significant assistance from the Division and that it will therefore will not provide diabetes-related aids or services for students with diabetes who will participate in that activity, it will notify the parents or guardians of these students at least three school days before the activity is to take place and will provide them with notice of their rights, including the right to file a grievance with the Division or a complaint with OCR, and information on who to contact in connection with such a filing.

Activities Covered. I, the Participant, understand that this agreement includes all activities, events or services (hereinafter referred to as "BOOTPRINTS HIKING TOURS") provided, arranged, organized, conducted, sponsored or authorized by BOOTPRINTS HIKING TOURS INC. and its directors, officers, employees, agents, guides, instructors, independent contractors, sub-contractors, representatives, sponsors, and their family, heirs, successors and assigns (all of whom are hereinafter collectively referred to as "THE RELEASEES") and shall include, but is not limited to guided hiking tours, transfers to/from tour sites and accommodations, orientation and instructional courses, overnight camping, meals and snacks, sightseeing, and other such activities, events and services in any way connected with or related to the participation in activities contemplated by this agreement.

Activities Covered. I, the participant’s parent or guardian, and the participant if not a minor, understand that this agreement includes all activities, events or services occurring during the Waldorf Senior Marine Biology Week on Hermit Island, Maine in September of 2019 which is organized by Berkshire Waldorf High School referred to in this document as the “program”, and which includes but is not limited to hiking, tours, lessons, labs, social gatherings, and instructional courses, and overnight camping.
