Agreement Documentation. ‌ The contract between the Parties will consist of this final, executed Agreement, including the following Exhibits to the Agreement: (1) The document entitled “HHSC RFP ▇▇▇-▇▇-▇▇▇▇” (incorporated by reference as Exhibit A to this Agreement); (2) The document entitled “CONTRACTOR’S Proposal” (incorporated by reference as Exhibit B to this Agreement); (3) The document entitled “HHSC’s Uniform Contract Terms and Conditions, Version 1.4.1, with Special Condition 16.01(incorporated by reference as Exhibit C to this Agreement)”; and (4) The document entitled “Uniform Grant Management Standards Governor’s Office of Budget and Planning (As Adopted June 2004)”, (incorporated by reference as Exhibit D to this Agreement). (5) The document entitled “Additional Contract Requirements”, (attached hereto and incorporated as Exhibit E to this Agreement). Agreement Documentation. The Agreement between the Parties will consist of this executed contract document (the "EAP Services Agreement”); HHSC's Uniform Terms and Conditions (UTCs), Version 2.15; the HHSC Special Terms, the Required HHSC Forms, Exhibits, and Certifications; the RFP, as clarified by Questions and Answers and Addenda officially issued by HHSC; CONTRACTOR's proposal dated November 26, 2018, as modified by its submissions dated January 11, 2019; January 16, 2019; January 22, 2019; and January 24, 2019; and all agreed modifications to the above-referenced documents.

Agreement Documentation. The Agreement between the Parties consists of: ELIGIBILITY SUPPORT SERVICES AGREEMENT HHSC CONTRACT NO. 529-14-0006-00001 (1) This final executed document entitled “Eligibility Support Services Agreement between the Health and Human Services Commission and MAXIMUS, Inc.,” including Exhibits A through F attached to this document and listed below. This document and all Exhibits are referred to collectively as the Signature Document. The following Exhibits are incorporated into the Signature Document: a. Exhibit A: the Revised Terms and Conditions; b. Exhibit B: the Data Use Agreement; c. Exhibit C: the Revised Proposal; d. Exhibit D: Key Performance Requirements; e. Exhibit E: Deliverables; f. Exhibit F: Payment Schedules; and g. Exhibit G: Security and Privacy Initial Inquiry. (2) The Proposal submitted by CONTRACTOR dated April 24, 2015 which is incorporated hereto by reference as Attachment 1 (the Proposal); and (3) HHSC Request for Proposals for Eligibility Support Services #:▇▇▇-▇▇-▇▇▇▇ dated December 22, 2014 including all addenda, which is incorporated hereto as Attachment 2 (collectively, the RFP).

Agreement Documentation. The Agreement includes: (a) this Agreement, including Schedule 1 - Form of Services Authorization, Schedule 2 - Form of Change Order, and any other schedule/attachment affixed at time of execution; (b) any certificates of insurance or clearance; (c) any executed Services Authorization(s); and (d) any amendments executed in accordance with Section 1.01.

Agreement Documentation. The Agreement consists of the main Agreement document and the following appendices. In the event of conflict between the documents, the main Agreement document takes precedence over the appendices, and the appendices apply in the order below.
