Application for Reclassification. (a) An employee who claims that he is normally and regularly assigned work that is not appropriate to the job description for his classification shall make application in writing to his Supervisor for a change of classification on the form provided. Such application shall be made in duplicate and the Supervisor shall record the date of receipt and sign it. (b) If the supervisor fails to comply with the time limit as stated in Section 29.02(c) hereof, any change of classification arising from the application shall be retroactive to the start of the pay period following the third day after the Supervisor received the application. (c) Within three (3) working days, the Supervisor shall interview the employee, record his disposition on the application, sign both copies and dispose of them as follows: (i) If the request is granted, one copy shall be returned to the employee and one copy submitted to payroll immediately. The immediate change of classification shall be effective on the start of the pay period following the day of granting the change of classification by the Supervisor. (ii) If the request is not granted one copy will be returned to the employee and one copy passed to the Personnel Department. (d) If the request is not granted the employee may, within five (5) working days of receipt of the Supervisor's disposition, file a grievance in writing and proceed with the Grievance Procedure beginning at Step 2. (a) A request by an employee for an upgrading in his occupation to an available vacancy, or to a future vacancy, shall be made in writing in duplicate on a form provided and presented to his Supervisor, who shall record the date and acknowledge receipt of same. Within three (3) working days the Supervisor shall interview the employee, record his disposition, sign both copies and return one (1) copy to the employee. (b) If the Supervisor grants the request the change of classification shall become effective on the start of the pay period following the date the employee fills the vacancy.
Application for Reclassification. 12.2.1 An application for reclassification may be initiated by the employee or his/her immediate supervisor in writing on the appropriate Human Resources form which has been mutually agreed to by Local 1904 and the University. Attachments to the form must include: a.) Any and all job descriptions under which the employee has worked while in this title. If the employee does not have the job description, he/she may request copies from the supervisor. b.) A listing of any new and/or additional job duties not included in the prior job descriptions(s) as well as identification of any of the duties from the prior job description(s) no longer being performed. 12.2.2 Material/information which may be included by the employee beyond that which is set forth above is that which provides additional explanation as to how the scope of work and/or level of responsibility/authority has changed; how the specific duties and responsibilities of the position do not conform to the job description for the title, and/or which otherwise supports the employee's request for a reclassification. 12.2.2.1 An application for reclassification may be submitted at any time that the employee believes his/her duties and responsibilities have changed to a sufficient extent as to warrant a reclassification.

  • Adjustments for Reclassification, Exchange or Substitution If the Common Stock issuable upon conversion of this Note at any time or from time to time after the Issuance Date shall be changed to the same or different number of shares of any class or classes of stock, whether by reclassification, exchange, substitution or otherwise (other than by way of a stock split or combination of shares or stock dividends provided for in Sections 3.6(a)(i), (ii) and (iii), or a reorganization, merger, consolidation, or sale of assets provided for in Section 3.6(a)(v)), then, and in each event, an appropriate revision to the Conversion Price shall be made and provisions shall be made (by adjustments of the Conversion Price or otherwise) so that the Holder shall have the right thereafter to convert this Note into the kind and amount of shares of stock and other securities receivable upon reclassification, exchange, substitution or other change, by holders of the number of shares of Common Stock into which such Note might have been converted immediately prior to such reclassification, exchange, substitution or other change, all subject to further adjustment as provided herein.

  • Reclassification, etc If the Borrower at any time shall, by reclassification or otherwise, change the Common Stock into the same or a different number of securities of any class or classes, this Note, as to the unpaid principal portion thereof and accrued interest thereon, shall thereafter be deemed to evidence the right to purchase an adjusted number of such securities and kind of securities as would have been issuable as the result of such change with respect to the Common Stock immediately prior to such reclassification or other change.

  • Reclassification Procedure a. A completed Position Description Form and written explanation for a proposed reclassification request shall be submitted to the Agency Personnel Office. b. Agency shall review and verify the duties assigned to the position. Within thirty (30) days after receipt of reclassification request, the Agency shall notify the Union of its findings. If the findings indicate reclassification, the Agency shall decide to seek approval if necessary or remove duties.

  • Recapitalization or Reclassification If the Company shall at any time effect a recapitalization, reclassification or other similar transaction of such character that the shares of Common Stock shall be changed into or become exchangeable for a larger or smaller number of shares, then upon the effective date thereof, the number of shares of Common Stock which Holder shall be entitled to purchase upon Exercise of this Warrant shall be increased or decreased, as the case may be, in direct proportion to the increase or decrease in the number of shares of Common Stock by reason of such recapitalization, reclassification or similar transaction, and the Exercise Price shall be, in the case of an increase in the number of shares, proportionally decreased and, in the case of decrease in the number of shares, proportionally increased. The Company shall give Holder the same notice it provides to holders of Common Stock of any transaction described in this Section 5(b).

  • Reclassification, Exchange, Combinations or Substitution Upon any event whereby all of the outstanding shares of the Class are reclassified, exchanged, combined, substituted, or replaced for, into, with or by Company securities of a different class and/or series, then from and after the consummation of such event, this Warrant will be exercisable for the number, class and series of Company securities that Holder would have received had the Shares been outstanding on and as of the consummation of such event, and subject to further adjustment thereafter from time to time in accordance with the provisions of this Warrant.
