The Crew List clause requires the party responsible for providing a crew—such as a vessel owner or operator—to supply a detailed list of all crew members assigned to a project or voyage. This list typically includes names, positions, qualifications, and other relevant information about each crew member, and may need to be submitted before the commencement of operations or upon request. Crew List. The Company shall submit to the Union upon request a crew list of the seamen coming within the scope hereof showing the following information: (a) Seamen’s names. (b) Identity card or passport numbers. (c) Nationality. (d) Ranks and certificates of competency.

(4) The Occupational Accident Insurance Fund may require of the master and the shipowner at any time that 1. the crew list, 2. a copy of the crew list, or 3. an excerpt from the logbook reflecting the crew of the ship be submitted or transmitted for the purpose of verifying the maintenance of safe ▇▇▇▇▇▇▇ under the applicable provisions. The master and the shipowner shall be obliged to comply with the request in accordance with sentence 1 without delay.

The template must be filled out in its entirety by Owners and resubmitted electronically in Microsoft Excel format to the Charterer. Scanned lists are unacceptable.
