Default Enrollment Clause Samples
Default Enrollment. 16 1. New full-time employees who fail to submit a timely application to 17 Opt-Out or enroll into the medical and dental benefit plans described in Section A will 18 be enrolled by default in the County’s Major Medical PPO plan and Delta Dental plan, 19 with employee only coverage. Eligible dependents of such employees may be 20 enrolled in the default plans if the employee submits application requesting 21 dependent enrollment within fifteen (15) days of date default enrollment notice is 22 issued.
23 2. New part-time employees who fail to submit a timely application 24 to Opt-Out or enroll into the medical and dental benefits plans described in Section A 25 above will be enrolled by default in the County’s Major Medical plan, with employee 26 only coverage. Eligible dependents of such employees may be enrolled in the default 27 plan if the employee submits application requesting dependent enrollment within 28 fifteen (15) days of date default enrollment notice is issued.

Eligible dependents of such employees may be enrolled in the default plans if the 23 employee submits application requesting dependent enrollment within fifteen (15) days of 24 receiving notice of his or her default enrollment.

New Three-Quarter-Time and Half-Time employees who fail to submit 26 a timely application to Opt Out or enroll into the medical and dental benefit plans described in 27 Section A.2.b will be enrolled by default in the County’s Major Medical plan, with employee 28 only coverage. Eligible dependents of such employees may be enrolled in the default plan if 29 the employee submits application requesting dependent enrollment within fifteen (15) days of 30 receiving notice of his or her default enrollment.
Default Enrollment. 2.4.9.1. The Contractor shall request and subsequently receive EOHHS’s advance approval in writing prior to using the Default Enrollment process established by CMS in 42 CFR §§ 422.66(c), 422.68(a), and Section 40.1.5 of the Medicare Advantage and Part D Enrollment and Disenrollment Guidance. EOHHS may revoke its approval for a Contractor to conduct Default Enrollment at any time.
Default Enrollment. An enrollment process that allows a Medicare Advantage organization (MAO), following approval by the state and CMS, to enroll a MassHealth Member enrolled in an affiliated Medicaid MCO (i.e., MassHealth health plan) into its Medicare Dual Eligible Special Needs Plan (D-SNP) when that Member becomes newly eligible for Medicare (Parts A and B)– unless the Member chooses otherwise. This process is only permissible in circumstances where the Member enrolls with the One Care plan offered by their MassHealth health plan upon receiving Medicare eligibility. The Default Enrollment effective date shall always be the date of the individual’s first entitlement to both Medicare Part A and Part B.

Eligible dependents of such employees may be enrolled in the 30 default plans if the employee submits an application requesting dependent enrollment 31 within fifteen (15) days of receiving notice of his or her default enrollment.

Default Enrollment. Except for POA and IAFF employee groups, all new hire Benefits-Eligible (Permanent) employees who fail to make their elections within 30 days of their hire date will automatically be enrolled in the City’s least costly medical plan with Employee Only coverage. POA and IAFF employees will be automatically enrolled in the ▇▇▇▇▇▇ “Employee Only” plan. During Open Enrollment, employees who do not complete enrollment within the Open Enrollment period will have their current medical and life insurance automatically continued into the next Plan year as if the Employee elected to keep them. All other coverage, including Health Care and Dependent Care Flexible Spending Accounts will stop. Eligible non-IAFF and non-POA employees may have eligible Flex Plan Allotment funds remaining after the health coverage election placed in the taxable Cash Payment Option as provided in the employee’s Compensation Summary or Memorandum of Understanding (MOU).
