Homeless Management Information System Sample Clauses

Homeless Management Information System. Contractor must be in good standing in collecting and entering current, accurate, and comprehensive data that reflects the homeless prevention and intervention services delivered by Contractor into the Homeless Management Information System (HMIS), as defined by HUD, as a condition of funding under this Agreement. HMIS requirements are further described in Exhibit C.

Homeless Management Information System. (HMIS). Contractor and its employees working on this project shall participate in trainings as required by the County and must utilize the Homeless Management Information System.

Homeless Management Information System. The SNH CoC Board is responsible for approving the comprehensive homeless plan and will oversee the coordination of regional homeless activities. The plan addresses provisions for data collection. The SNH CoC Board agrees to commit funds on a population-based formula for the duration of this Interlocal Agreement (July 1, 2020, through June 30, 2021), not to exceed the amounts stated in Exhibit A, Interlocal Budget for the Homeless Management Information System (HMIS). The County will work with the Cities, providers, and the public to continue to incorporate these activities as a priority in the comprehensive homeless strategic plan to improve housing and service programs for the homeless in Clark County (among public, private, and non-profit jurisdictions), address competing goals and conflicting services, and reduce/eliminate duplication of services.

Homeless Management Information System. Subrecipient must enter all appropriate and necessary data into Homeless Management Information System (“HMIS”) at the time of client intake, if applicable, or at such other times required, or require subcontractors providing the Services for which funding has been provided under this Agreement to enter. Exceptions are only allowed with prior written approval provided from State via COIC.

Homeless Management Information System. Contractor shall enter and maintain data in the RTFH approved Homeless Management Information (HMIS) system as directed by RTFH. Contractor will comply with the HMIS Policies and Procedures in effect during the period of this Agreement including those for data collection, data entry, data quality, and standards for missing data, incomplete data, and timeliness of data entry.

Homeless Management Information System. A. SUBRECIPIENT agrees to participate in the Homeless Management Information System (HMIS). 1. Participation is defined by HMIS training attendance, complying with Riverside County HMIS security policies and procedures, data collection, and entering required client data on a regular and timely basis. 2. COUNTY retains the rights to the HMIS and case management software application used in the operations of this property. COUNTY will grant SUBRECIPIENT access to use the HMIS software for the term of this Agreement. 3. SUBRECIPIENT agrees to provide BCSH access to HMIS data collected and entered into the SUBRECIPIENT’S HMIS, upon request, and to participate in any statewide data initiative as directed by BCSH, including, but not limited to, a statewide data integration environment.

Homeless Management Information System. 10.1 System-wide HMIS. The CoC shall designate a single Homeless Management Information System (hereinafter “HMIS”) to operate system-wide.

Homeless Management Information System. As stated in the Sonoma County Homeless Management Information System Participant Agreement, SUBRECIPIENT must be in “good standing” in collecting and entering current, accurate, and comprehensive data that reflects the homeless program services delivered by SUBRECIPIENT into the COUNTY’s Efforts to Outcomes (EtO) Homeless Management Information System (HMIS) licensed by Social Solutions Group as a condition of funding under this Agreement.

Homeless Management Information System. The Contractor agrees to fully comply with the Rules and Regulations required by the U.S. Dept. of Housing and Urban Development (HUD) which governs the Homeless Management Information System (HMIS). HUD requires recipients and sub recipients of ▇▇▇▇▇▇▇▇-▇▇▇▇▇ Act funds to collect electronic data on their homeless clients through HMIS. Programs that receive funding through ▇▇▇▇▇▇▇▇-▇▇▇▇▇ that produce an Annual Progress Report (APR) must also collect program level data elements. These programs include but are not limited to: Continuum of Care (CoC), Section 8 Mod Rehab, Emergency Solutions Grant (ESG), and Housing Opportunities for Persons With AIDS (HOPWA). Project types include, but are not limited to: Emergency Shelter, Transitional Housing, Rapid Rehousing, Diversion, Permanent Housing, Supportive Services, and Street Outreach. Participation in HMIS is a requirement for recipients of City of Denver homeless funding. The Contractor, in addition to the HUD requirements, shall conform to the HMIS Policies and Procedures established and adopted by the Metro Denver Homeless Initiative (MDHI) Continuum of Care. Technical assistance and training resources for HMIS are available to the Contractor via the Colorado HMIS Helpdesk based on requests by the Contractor to DHS and by periodic assessments of participation, compliance, and accuracy of data collection.
