Common use of Installation SLA Clause in Contracts

Installation SLA. Customer must make a written request (e-mail or fax) to the Verizon Account Team within thirty (30) days after the date the Managed Device installation is completed with the following information:  The Site identifier  The date the parties agreed that the Managed Device should have been installed  The date the Managed Device was installed  The date that the SOR was executed by both parties
