The LEAVE PROGRAM clause establishes the terms and conditions under which an employee may participate in, or exit from, a company's leave program. Typically, this clause outlines eligibility requirements, the process for applying for leave, and the procedures for returning to work or withdrawing from the program. LEAVE PROGRAM. The Catastrophic Leave Bank (C.L.B.) is a “bank” of donated sick leave days which may be requested for use by Unit Members who are suffering from a catastrophic illness or injury, once they have exhausted all full-paid leaves.
5.19.1 A Unit Member is eligible to use Catastrophic Leave when the following conditions are met:
a) The Unit Member requesting use of the leave has donated leave to the program pursuant to the minimum contribution (see 5 below) requirements set forth in this procedure prior to requesting leave;
b) The Unit Member has submitted a written request that includes a physician’s verification of a severe or incapacitating illness or injury that is expected to continue for an extended period of time and prevents the Bargaining Unit Member from performing his/her duties;
c) The Unit Member will not receive payment for the time absent due to illness or injury from Workers’ Compensation (C.W.C.I.A.);
d) The Unit Member has exhausted all balances in all full-paid leave categories;
e) The Unit Member has not withdrawn more than forty (40) ays of sick leave per catastrophic illness or injury;
f) There is sufficient leave in the C.L.B.
5.19.2 Enrollment in Catastrophic Leave Bank
a) Current Unit Members: An annual open enrollment period will occur each March; however nothing precludes any C.L.B. member from donating to the bank at any time.
b) New Hires: Newly hired Unit Members may donate one day of sick leave of the C.L.B. immediately upon hire. After the initial enrollment, the Unit Member must maintain the required balance in order to donate.
LEAVE PROGRAM. Permanent employees represented by the SEIU Public Works Unit, may be eligible to receive donations of paid leave, to be included in the recipient employee’s sick leave balance if he/she has suffered a catastrophic illness or injury which is defined as a serious medical condition considered to be terminal, a major physical impairment, or a family medical emergency (defined as a catastrophic illness or injury of a spouse, registered domestic partner, child or parent residing in the employee’s household), subject to the following conditions: • The recipient employee, recipient employee’s family, or other person designated in writing by the recipient must submit a request to the City Administrator or designee. • The recipient employee is not eligible so long as he/she has paid leave time available; however, the request may be initiated prior to the anticipated date that all leave balances will be exhausted. • The recipient employee must provide a medical verification which meets the criteria above and a prognosis. • Donations may be made in whole hour increments, and are irrevocable. The donor employee may donate vacation up to any amount, but the City may require that the donor employee retain 40 hours of vacation leave. Sick leave may be donated up to 24 hours in whole hour increments. Donations may be made from any individual employed by the Public Works Department. • Time donated will be converted from the type of time donated to sick leave and credited to the recipient employee’s sick leave balance on an hour-for-hour basis and shall be paid at the rate of pay of the recipient employee. • Time donated in any pay period may be used in the following pay periods. No retroactive donations will be permitted. • Eligibility for this program requires recommendation by the Public Works Director and approval by the City Administrator.
LEAVE PROGRAM. The Court will follow the catastrophic leave program of the City and County of San Francisco.

LEAVE PROGRAM. 282. Covered employees may participate in the catastrophic leave program administered by the City and County of San Francisco pursuant entirely to its provisions.
LEAVE PROGRAM. The Catastrophic Leave Program is for employees who have exhausted all accrued leaves due to a serious or catastrophic illness, injury, or condition. The Catastrophic Leave Program allows employees to donate time to any qualifying employee so that he/she can remain in a paid status for a longer period of time, thus partially ameliorating the financial impact of the illness, injury or condition.
LEAVE PROGRAM. The District and the Association agree to the establishment of a Classified Unit Member Catastrophic Leave Program (Catastrophic Leave Program). The intent of this Catastrophic Leave Program is to provide additional financial protection to those unit members who incur a period of prolonged illness/injury or hospitalization.
LEAVE PROGRAM. The provisions of this ARTICLE shall apply only to regular, full-time employees in the classified service. Employees appointed to certain grant funded positions as designated by the Anaheim City Manager under Section 22.8 and newly hired probationary employees shall be excluded from the provisions of this ARTICLE.
LEAVE PROGRAM. The granting of leave shall be in writing and shall be approved by the Sheriff, or designee, prior to the leave being taken, except in the case of an emergency. Said leave shall be granted if the member has sufficient time accrued, unless this would create undue hardship on the remaining scheduled personnel, as in the case of too many other personnel already scheduled for leave time. When prior approval for leave cannot be obtained by the member due to an emergency, the Shift Sergeant will be notified, and a leave request filed immediately.
LEAVE PROGRAM. 1. When a catastrophic illness or injury incapacitates a unit member or a member’s child, spouse, parent or other relative living within the unit member’s immediate household for an extended period of time, fellow unit members may donate one (1) to three (3) accrued sick leave credits to that unit member. Donations made shall be strictly voluntary.
2. A unit member who is, or whose child, spouse, parent or other relative living within the unit member’s immediate household is, suffering from a catastrophic illness or injury may request donations of accrued sick leave credits. Upon requesting donations, the unit member shall provide a doctor’s verification of the catastrophic injury or illness.
3. The Superintendent, and/or appropriate administrator shall determine:
a. That the unit member is unable to work due to the unit member’s or his/her family member’s catastrophic illness or injury, and
b. That the unit member has exhausted all applicable accrued paid leave credits.
4. When the above verification and determinations are made, the Superintendent and/or appropriate administrator may approve the transfer of accrued sick leave credits. All transfers of sick leave days are irrevocable.
5. The Superintendent and/or appropriate administrator shall inform unit members of the means by which donations may be made in response to the unit member’s request. A limit of 150 days of sick leave credit may be held for future use.
6. Any unit member, upon written notice to the district, may donate accrued sick leave credits to the requesting unit member at a minimum of one full work day, and in full work day increments thereafter. All transfers of eligible leave credit shall be irrevocable.
a. One full work day of sick leave donated counts as one full work day received.
7. To ensure that unit members retain sufficient accrued sick leave to meet needs that normally arise, donors shall not reduce their accumulated sick leave to fewer than fifteen (15) days.
8. When a unit member retires or otherwise ends employment with the District with a balance of donated leave pursuant to paragraph 5 above, said leave balance dissolves.
J. All other leave provisions not covered herein shall be consistent with the provisions of the Education Code, i.e., sabbatical leave, military leave, etc.
