Leave Request. Any employee who has a claim for compensation while absent because 31 of injury or illness incurred as prescribed herein, shall file a claim on the standard leave 32 form provided by the Board with his/her principal or other immediate supervisor by the 33 end of the fifth working day following the employee’s return to duty after the leave for 34 illness or injury-in-line-of-duty.
Leave Request. A leave of absence without pay shall be granted to tenured teachers at their request for child bearing, for rearing of children up to age 6, or for caring of unmarried children who are mentally or physically incapable of caring for themselves thus requiring custodial care. Leave Request. 1. The employee will provide notice to the University that includes the anticipated dates of absence and the type of leave the employee wishes to use during the absence. When the request for leave is for medical reasons, the employee will also provide a doctor’s statement to the University.
2. The University will respond to the request for leave within ten (10) days. Requests for medical reasons will be expedited and approved as soon as possible.
3. If the request is denied, the employee will receive written notice, which includes a description of the operational necessity and procedures for the employee to appeal the denial.
4. The employee will have the right to appeal denial of the leave request. The appeal must be submitted within seven (7) days to the Chief Human Resource Officer. The Chief Human Resource Officer will review the appeal and will have the authority to overturn the denial. The employee will be allowed representation for preparing and presenting the appeal.
Leave Request. 7.7.1 Employees are required to complete “Request for Short Leave of Absence Forms”, Exhibits E(1) and E(2) and secure permission from the Division prior to taking any such leave, except in emergency situations.
