OUTCOME DATA. Charter School shall gather and maintain all data related to placement, tracking, and monitoring of student suspensions, expulsions, and reinstatements, and make such outcome data readily available to the District upon request. REHABILITATION PLANS Pupils who are expelled from Charter School shall be given a rehabilitation plan upon expulsion as developed by Charter School’s governing board at the time of the expulsion order, which may include, but is not limited to, periodic review as well as assessment at the time of review for readmission. Terms of expulsion should be reasonable and fair with the weight of the expelling offense taken into consideration when determining the length of expulsion. Therefore, the rehabilitation plan should include a date not later than one (1) year from the date of expulsion when the pupil may apply to Charter School for readmission. Charter School shall inform parents in writing of its processes for reinstatement and applying for expungement of the expulsion record.

OUTCOME DATA. Contractor shall be responsible for tracking and reporting on the following outcome data at intervals designated by State: a) Number of referrals; b) Number of offenders denied due to program exclusionary criteria; c) Number of offenders served in the Facility; d) Offender demographics; e) Average number of treatment contact hours; f) Average length of stay for program completers and non-completers; g) Successful completion, non-completion and termination rates; h) Behavior Management System applications; i) Verification of participant changes in knowledge, skills, and attitude using a State-approved tool; j) Follow-up to ascertain the degree of compliance with the transfer and aftercare plan, which focuses on continuity of care, life-style changes and a reduction in addictive criminal behavior; and
OUTCOME DATA. If necessary, the OAC shall require and collect additional outcome- related data from the Parties to evaluate the use of the Opioid Funds.
OUTCOME DATA. Provide any outcome data such as numbers of participants entering employment, types of employment found, wage rates, etc., collected by counties. • Please forward such data with the FSET County Plan. • Include a copy of any evaluations or studies relating to employment and training conducted or commissioned by counties.

  • Protocols Each party hereby agrees that the inclusion of additional protocols may be required to make this Agreement specific. All such protocols shall be negotiated, determined and agreed upon by both parties hereto.
