Outcome Reporting. LLS may contact the Grantee after the conclusion of the Grant to determine how LLS funding influenced his/her career and how it may have contributed to new treatments, prevention or diagnosis for patients with hematologic malignancies and/or related pre-malignant conditions. Grantee agrees to cooperate with LLS in connection with such contact.
Outcome Reporting. Sponsoring Institution will cause Grantee to cooperate with LLS after termination of this Agreement to determine how LLS funding influenced his/her career and how it may have contributed to new treatments, prevention or diagnosis for the applicable condition(s).
Outcome Reporting. A. The Department is federally required to reduce or eliminate the administrative requirements and reporting burden put upon local health departments supporting grant activities, and the Department cannot request or require additional programmatic reports, work plans, or expenditure information from local health departments beyond what is required by the grant, unless otherwise required by law. Therefore, the outcome reporting for Strategy A1: Workforce, Strategy A2: Foundational Capabilities, and Strategy A3: Data Modernization include:
1. Quantitative Reporting
a. The Subrecipient shall report the outcome measurements listed in Section V. to the Department at a frequency and format as determined by the CDC; and

Sponsoring Institution shall cause all Key Sponsoring Institution Investigators to cooperate with LLS after termination of this Agreement to determine how LLS funding influenced his/her career and how it may have contributed to new treatments, prevention or diagnosis for patients with hematologic malignancies and/or related pre-malignant conditions.
Outcome Reporting. Contractor shall report on these outcomes quarterly during the regularly scheduled monitoring calls/activities.
Outcome Reporting. The Contractor's MIS shall have the capacity to report on the measures outlined in Section C.17.7.
Outcome Reporting. The AAA shall develop, implement, and maintain an internal quality management system that evaluates the AAA’s programs and establishes a system of self- correcting feedback that may be externally validated by DHHS.

5.1 DHHS Activities. DHHS shall:
(1) oversee the program in accordance with applicable federal regulations and the Utah State Medicaid Plan as it currently exists or is hereafter amended;
(2) provide, as necessary and requested, timely information, training and guidance to assist the Contractor to meet the responsibilities;
Outcome Reporting. Outcome reports will be submitted in the agreed format to NHS Liverpool Clinical Commissioning Group by the 15th working day of each month. In accordance with NHS Caldicott Principles you are requested to submit patient named data to LCCG Performance and Intelligence Team marked ‘Private and Confidential – to be opened by Addressee only” for paper records.
