Patient Consent. 7.1. The Practice is responsible for meeting privacy and consent requirements and obtaining specific patient consent to transfer health information to third parties for the purposes of data sharing under this agreement including quality improvement, clinical audit activities, and research. 7.2. The Practice acknowledges that the Software is managed on the basis of its individual patient’s implied consent for use of the de-identified information in accordance with Core Standard 3.6 and Core Standard 6.3 of RACGP’s Standards for general practices, 5th edition. 7.3. The Practice agrees to display an in-practice poster (similar to Appendix C) in a prominent position within their reception or waiting areas.

Patient Consent. Participant is responsible for obtaining or requiring that its Participant Members obtain any authorization or consent from any Individual whose Data it Transacts through the Interface unless otherwise agreed by the Parties.

Patient Consent. Participant is responsible for obtaining any and all necessary patient consents and authorizations relating to the use and exchange of patient information, including without limitation consent to release HIV test results, genetic test information, substance abuse information, and as otherwise required by law. The personal data will be deidentified (pseudonymized) to the furthest extent possible. Such scientific research, research and product development would initially be focused on improving process, image and digital planning quality and optimize workflow.

The patient confirms that his/her consent to the terms of this consent form is provided of his/her own free will without any undue influence from any person whatsoever. 4.8.6. The patient confirms that he/she has the permission of his/her dependent(s) to give their consent, where such consent has been provided and indemnifies the Optometrist against this.

Patient Consent. Training Center shall ensure that all necessary authorizations that may be required under any applicable state, federal or local laws, regulations or and ordinances are obtained from patients prior to allowing any Accuray employees or Customers to observe the treatments of such patients.
