PROVIDER STANDARDS. 1. All services rendered by the Contractor shall be consistent with customary standards of professional practice in amount, duration, scope, and quality. 2. The Contractor, and its employees and agents who are providing services under this Agreement, shall be qualified, licensed or certified or both in their respective disciplines as required by the Commonwealth of Pennsylvania and meet staffing standards as required by the Commonwealth of Pennsylvania and their respective accrediting body or waiver agreement.
PROVIDER STANDARDS. A. The Provider shall comply with and not violate the corporate practice of medicine doctrine. B. All services rendered by the Provider shall be consistent with customary standards of professional practice in amount, duration, scope and quality. C. The Provider, and its employees and agents who are providing services under this Agreement, shall be qualified, licensed and/or certified in their respective disciplines as required by the Commonwealth of Pennsylvania. D. The Provider, and its employees and agents who are providing services under this Agreement, shall at all times be enrolled as Pennsylvania Department of Human Services Medical Assistance (Medicaid) providers and as U.S. Social Security Administration Medicare providers in good standing, in order to participate in the CRDP and to be paid for services provided under the terms of this Agreement. The Provider shall notify the Department in writing within 15 calendar days of the Provider’s preclusion or exclusion from participation in the Medicaid or Medicare programs. The Provider agrees that if precluded or excluded, voluntarily or involuntarily, from Medicaid or Medicare Programs, this Agreement shall terminate immediately as of the date of such preclusion or exclusion. The Provider further agrees to notify the CRDP in writing of any misdemeanor or felony conviction relating to a Medicaid or Medicare practice offense by the Provider or any of its owners, agents, or employees within 15 calendar days of conviction. Likewise, the Provider shall notify the CRDP in writing of any professional licensing board action against any health care professional employed or utilized by the Provider in providing services to CRDP patients under this Agreement within 15 calendar days of action. E. The Provider agrees that, in the event it refers patients to other providers, it shall refer Medicaid or Medicare-enrolled patients only to Medicaid or Medicare-enrolled providers.
PROVIDER STANDARDS. Provider hereby warrants and represents that the information contained in the application of Provider to become a Participating Provider is true and correct in all respects and does not fail to state a material fact without which the information would be misleading.
PROVIDER STANDARDS. The Supplier undertakes to fully comply with the guidelines established in this document and to report any prior or subsequent non-compliance within 5 business days following the date of signature of this document or the date on which it becomes aware of the non-compliance with these guidelines through the FUNO ethics line mentioned below. The purpose of these guidelines is to ensure Supplier's compliance with our business practices and the guidelines of our code of ethics that at the signing of this document it acknowledges having read, as well as its compliance with applicable legislation at the national and local level, in terms of the following:
PROVIDER STANDARDS. A. The Provider shall assure that its owners, employees and agents shall be qualified, and licensed as required by the Commonwealth of Pennsylvania. B. In order to be eligible for reimbursement of non-emergency ambulance services, (see Appendix C) the Provider shall use independent and hospital-owned non- emergency ambulance services enrolled as Pennsylvania Department of Public Welfare Medical Assistance (Medicaid) and U.S. Department of Health and Human Services Medicare certified providers, and be in compliance with Federal and State laws and regulations pertaining to the provision of non-emergency ambulance service. Such non-emergency ambulance services must be provided by an entity in compliance with 28 Pa.Code Part VII or subsequent amendment, for continued participation in the CRDP. The Provider shall not be eligible to receive CRDP reimbursement and shall not be reimbursed by the CRDP for non- emergency ambulance service provided as of the date the non-emergency ambulance service ceases to be a Medicaid or Medicare provider.

  • OMB Standards Unless specified otherwise within this agreement, the Subrecipient shall procure all materials, property, or services in accordance with the requirements of 24 CFR 200.318, and shall subsequently follow, Property Management Standards, covering utilization and disposal of property.

  • Contractor Standards Contractor shall comply with Contractor Standards provisions codified in the SDMC. Contractor understands and agrees that violation of Contractor Standards may be considered a material breach of the Contract and may result in Contract termination, debarment, and other sanctions.

  • Policies, Guidelines, Directives and Standards Either the Funder or the Ministry will give the HSP Notice of any amendments to the manuals, guidelines or policies identified in Schedule C. An amendment will be effective in accordance with the terms of the amendment. By signing a copy of this Agreement the HSP acknowledges that it has a copy of the documents identified in Schedule C.
