Quarterly Performance Reports due on the 15th of January, April, July, and October of the preceding three (3) months (i.e., the July report covers the months of April, May, and June). Reports shall address all Project activities described in the Work Statement. Failure to submit timely Quarterly Performance Reports will result in suspension of reimbursement of funds requested until all reports are brought current.
Quarterly Performance Reports. Quarterly performance reports will be required to be completed during the first year of reemployment on any EMPLOYEE appointed from a reemployment list or classification reinstatement list.
Quarterly Performance Reports. Pursuant to Section 3 of this Agreement, IIM shall provide the following reports on a calendar quarter basis:
Quarterly Performance Reports. The Contractor shall provide quarterly performance reports to the Department addressing the Contractor’s compliance with the performance measures, as applicable, and as stated in Section II., DD., of the Contract. If issues of non-compliance are identified in the quarterly performance reports, financial consequences will be assessed in accordance with Section II., EE., of the Contract. The quarterly performance reports shall be submitted by the tenth (10th) day of the month following the end of the corresponding quarter.
Quarterly Performance Reports. Results of the quarterly performance standards shall be reported to the BOARD within 60 DAYS of the end of the reporting period.
Quarterly Performance Reports. The Recipient shall submit quarterly performance reports to the USAID AOR to reflect results and activities of each preceding quarter. Reports are to be submitted within 15 days of the end of each quarter. These reports shall summarize the outcomes of the Recipient’s activities during the particular reporting period, document any program accomplishments or progress towards results during the reporting period, compare those results to the planned tasks in the implementation plans and M&E Plan, and discuss any potential constraints that might prevent the Recipient from meeting agreed upon targets and benchmarks. Reports should also contain, as an attachment, a list of all subgrants issued under the award during the reporting period. The list should contain the name and contact information for each subgrantee, the title and duration of the program, the amount of the award, and a brief description of the program. The quantitative results should be presented in quarter as well as aggregated format to show the progress towards the annual targets. Quarterly reports should also include any additional qualitative information the Recipient would like to include to demonstrate the results achieved vis-à-vis the project’s objectives during the reporting period. The reports may be submitted by email and shall contain the following information at a minimum:
a) An analytical description of overall program progress toward results that reflects and synthesizes achievements of all partners. This should not be a description of activities from each partner, but rather a broader analysis that examines the progress of all partners in the context of program objectives and expected results.
b) Data on all indicators established in the Performance Monitoring and Evaluation Plan. Data should be disaggregated by gender and other historically disenfranchised populations where relevant.
c) A comparison of accomplishments to the goals and objectives established for the period.
d) Problems encountered, reasons why established goals were not met, if appropriate, and how challenges or problems will be overcome during the next reporting period.
e) Priorities for programming during the next reporting period. The quarterly report covering the 4th quarter of each fiscal year will provide USAID with annual data on the agreed upon performance indicators. The fourth quarterly report shall be due by the end of the fiscal year – by September 30 of each year.
Quarterly Performance Reports. The Recipient shall submit quarterly, cumulative, Performance Reports to the Human Services Department of the County within fifteen (15) business days following: December 31, March 31, June 30, and September 30. These reports should include but not limited to the number of unduplicated children served during the quarter, and the progress the agency has made toward meeting their goals and objectives as they stated in their RFP response.
Quarterly Performance Reports. (i) Quarterly reports shall indicate the following information as a minimum:
i. percentage of both the number loans to and total investment into Companies Assisted;
ii. internal leverage ratio for ECGRA Funds;
iii. total internal leverage raised, including source and terms of internal leverage funds;
iv. job creation/retention attributed to loans made pursuant to this Agreement;
v. list of borrowers by organization’s name & contact person (to be kept confidential unless a waiver is received);
vi. job creation/retention attributed to loan recipients that have signed a confidentiality waiver; and,
vii. total company investment leverage attributed to loan recipients that have signed a confidentiality waiver.
(ii) Quarterly performance reports shall be provided to ECGRA for each calendar quarter, and shall be due 45 days following each quarter ending March 31, June 30, September 30 and December 31.
(iii) Quarterly performance reports shall be provided to ECGRA until such time that this Agreement has terminated or the satisfaction of Levels of Accomplishment, which ever shall occur last.
Quarterly Performance Reports. These quarterly reports will detail the prior period’s progress on implementing the Agreement Addendum and shall include the county’s number of current active cases and contacts, how many LTBI cases initiated medication, how many LTBI cases completed medication, and when having provided DOT and DOPT, how many are provided face-to-face and how many are provided electronically. The first report is to report for June, July, and August 2022 activities and is due by September 22, 2022. The periods for these reports are defined as: • June – August 2022 • September – November 2022 • December 2022 – February 2023 • March – May 2023
Quarterly Performance Reports. The Grantee will submit quarterly performance reports to the County not later than 30 calendar days after the reporting period as determined by the County. Quarterly performance reports must include the following information:
i. A cover letter addressed to the Department’s Bureau Chief of Strategic Planning and Policy, including the name of the Project and its associated section number;
ii. An estimated percentage of construction work completed for the Project;
iii. A statement indicating whether construction of the Project is on, behind or ahead of schedule;
iv. A record of construction activities and expenditures to date and for the current reporting period;
v. A forecast of quarterly construction activities and expenditures for the remainder of the Project; and
vi. Any significant changes to the Project schedule.
