Service Coordinator. The person with primary responsibility for providing Service Coordination and Care Management to STAR+PLUS MMP Enrollees. The Service Coordinator leads the Service Coordination Team and actively collaborates with the Enrollee’s specialty care Providers, including behavioral health and LTSS service Providers, as appropriate.
