SERVICE USER NEEDS. 5.1. In advance of the start of the service [Effective Date] the Purchaser shall supply to the Provider copies of any up to date assessments of Service User needs, to assist the Provider in drawing up a Care Plan. 5.2. The Care Plan shall be developed in accordance with Person-Centred principles and shall set out in detail how the Provider shall meet the assessed needs of the Service User, including specialist needs. This will be shared with the Service User and all relevant parties such as carers and other professionals where appropriate. 5.3. The Provider shall leave a copy of the Care Plan and relevant risk assessment documentation in the Service User‟s home and shall make available a copy to the Health and Care Point within one week of the Effective date if requested. 5.4. An initial review shall be convened by the Provider of each Service User‟s needs within 3 months of the Effective Date to establish the appropriateness of the Service. The Service User should be at the centre of this process which should focus on what‟s working and not working and lead to action. 5.5. Following this initial review, the Provider shall undertake regular reviews of at least every six months of Service User‟s needs as set out in the Care Plan and shall give 24 hours advance notice to the Health and Care Point of any recommended changes in a Service User‟s needs that in the Provider‟s, ▇▇▇▇▇‟s or the Service User‟s opinion necessitates a variation to the Request for Homecare Services. The Service User should be at the centre of the process and the provider should ensure that the Service User is appropriately prepared to attend the review. 5.6. The Provider shall also participate in reasonable formal multi-disciplinary reviews of a Service Users needs when requested to do so by the Purchaser , and shall comply with all reasonable requests for their staff to assist with those reviews. 5.7. The Provider shall not offer or provide to the Service User any services other than the Service as defined in this Agreement and in their Request for Homecare Services except where it is clearly distinguishable from the Services commissioned by the Purchaser, is at the Service User‟s request, and is not charged to the Purchaser. 5.8. The Provider shall deliver services as specified in the Request for Homecare Services.
