Termination Slip. An Individual Employer is required to provide the employee and the Union with a termination slip within one (1) working day of the termination of an employee (whether the employee resigns or is discharged, laid off or otherwise terminated for any reason). The termination slip shall state the reason for termination. When terminated for any reason, the employee shall register on the out- of-work list no later than three (3) working days from the date of termination.
