Card Issue. 1.1 The Bank shall issue under the Programme a Card to each employee of the Company as may from time to time be nominated by the Company’s authorised officers and accepted by the Bank. 1.2 The Bank shall deliver each Card to the Company which shall be responsible for ensuring that each Card is signed by the employee whose name is embossed on the Card (the “Cardholder”) immediately upon receipt. The Bank shall be under no responsibility to the Company to ensure that a Cardholder duly complies with these terms and conditions or take any legal action or proceedings against a Cardholder. The Bank shall be under no responsibility to ensure that the Card is used for any business purpose or within any Cardholder’s authority conferred by the Company in respect thereof.
Card Issue. The Bank shall issue under the Programme, a charge card, being a World Corporate MasterCard (the "Corporate Card") and/or a Platinum Purchasing MasterCard (the "Purchasing Card") to each of its customers (the "Employer") and to each employee identified in the Commercial Card Programme Cardholder Nomination Form annexed hereto and to such employee as may from time to time be nominated by the Employer's authorised officers. Corporate Cards and Purchasing Cards are collectively referred to below as "Cards".
Card Issue. 6.1 By signing this Agreement, you ask us to issue a Card, and any additional, renewal and replacement Card. 6.2 You must be aged 18 years or over. The provision of this Agreement to you is not a commitment by us to make a credit card facility available to you. This Agreement is made when it is signed by you and on our behalf. 6.3 Unless and until any termination takes place, we shall issue you and any additional Cardholder with renewed cards from time to time.
Card Issue. 1. The Customer shall send via the Euronet Web portal the list of the Authorised Customer’s Employees (hereinafter “the Card Issue File”) in the form of the CSV file, according to § 1 of Appendix No 2 to T&C, along with the following information: a. full name; b. permanent address and residence address; c. citizenship and date of birth; d. Personal ID No (PESEL); e. if the Agreement specifies more than one Bank Account for Deposits – information as to which of such Accounts the Card of the Authorised Customer’s Employee is to be assigned. In case of lack of any of the above-mentioned data Euronet Project Manager shall address Customer Project Manager to supplement it and till such moment the Card Issue File shall not be processed. 2. If requested by Euronet, the Customer shall also send, to ▇▇▇▇▇▇▇▇▇▇▇▇@▇▇▇▇▇▇▇▇▇▇▇▇▇▇▇▇.▇▇▇, scans of ID documents of all Authorised Customer’s Employees listed in the Card Issue File. 3. Euronet shall be entitled to check the accuracy of the sent details, the compliance of the attached scans of documents of the Authorised Customer’s Employees as regards their format, legibility and veracity, also with the use of external data bases, and shall verify the presence of the Authorised Customer’s Employees on sanction lists. 4. Euronet shall confirm the accuracy and compliance of details included in the Card Issue File and scans of documents by sending a confirmation electronically to the e-mail address indicated in the Agreement. In case of finding any irregularity or inconsistency in the sent details or presence of the Authorised Customer’s Employees on sanction lists, Euronet has the right to refuse to issue a card to the Authorised Customer’s Employee or to request additional information or documents. In such a case, the Customer shall be notified of the Euronet’s request in the manner indicated in the first sentence of this section. 5. Once Euronet confirms the accuracy and compliance of details provided in the Card Issue File and the Customer delivers the missing details or documents requested by Euronet, Euronet shall deliver Cards in a manner agreed upon in the operating mode by the Project Managers, within 30 calendar days from the date of sending by Euronet a confirmation of the accuracy of documents and details included in the Card Issue File, in the number resulting from the Card Issue File, subject to clause 7 below. 6. Card Issue. III.1 The Bank will issue the Plastic Carrier to the Cardholder within 14 Banking Days of conclusion of the Agreement and the Virtual Card without undue delay after concluding the Agreement. Issue of the Card is understood to be its distribution to the Cardholder in the specified manner. The deadline for issuing the Card is met by the Bank provided the Bank sends the Plastic Carrier to the Cardholder within this period, makes the Virtual Card available via Mobile Banking, or allows the Cardholder to pick up the Plastic Carrier within this period. III.2 The Bank will assign Limits to the Card based upon agreement with the Account Owner. Card Limits may be changed in the manner specified in paragraph I.2 of the Terms. III.3 When automatically renewing the Card, the Bank is entitled to issue the Cardholder a different Card than the one used up until that point. If issue of a different Card represents a change in the contractual terms, the Account Owner approves of such change in the manner specified in paragraph I.2 of the Terms. III.4 The Card is the property of the Bank. III.5 The Card is valid up until the time designated on the front of the Card. The Card validity expires on the last day of the month designated on the front of the Card or as of the moment of activation of the Renewed Card replacing the original one. The Cardholder is entitled to Card Use as long as the Card is valid.

Card shall be issued to Cardholder, being an individual over 18 years of age, expressly specified by Customer in the Ap- plication being Customer himself in case of a natural person, or Customer’s authorised Cardholder, subject to Unlimint’s approval. 2.2. If Customer is a Corporate Customer, Customer certifies that authorised Cardholder specified in the Application to whom Card shall be issued represents Customer, being its director, representative, signatory, owner, employee. 2.3. Customer is obliged and bears responsibility for providing in the Application and during the validity of the Agreement ac- curate, correct and complete details, information and docu- ments on Customer and Cardholder, in accordance with re- quirements of Unlimint. 2.4. Unlimint is not responsible and bears no liability for losses/inconveniences sustained by Customer/Cardholder in case of Customer's failure to fulfil his obligations in the preceding clause and in case information provided to Unlim- int contains errors and/or omissions. 2.5. Unlimint may offer different types of Card Products from time to time. Customers should consult on the relevant Card Product/features in the lists of fees and charges for Cards. Unlimint may make available additional information/descrip- tions on its Website or at Customer’s request. 2.6. Unlimint may restrict or limit the type and/or number and/or features of Card Accounts and/or Cards and/or Card Prod- ucts that can be opened/provided/issued to Customer and/or Customer type or category and/or to one Cus- tomer/Cardholder. 2.7. Card Issue. The Bank may, at its sole discretion, issue under the Programme, a charge card, being a World Corporate MasterCard (the "Corporate Card") and/or a Platinum Purchasing MasterCard (the "Purchasing Card") to each of its customers (the "Customer") and to: (i) each individual identified in the Commercial Card Programme Cardholder Nomination Form annexed hereto, and (ii) such s) (the "Related Entities") as may from time to time be nominated by the Customer's authorised officers (collectively, the "Cardholder(s)"). For the avoidance of doubts, if a Customer requests a Card to be issued to an employee of its Related Entities, the Bank may decline to issue such Card and, if the Bank considers it appropriate, recommend that such Related Entities apply to be on-boarded as customers of the Bank under separate cards programmes.

A Card can be issued to either the Individual Customer him- self (if a natural person) or to a Cardholder selected by the Corporate Customer in the Application, provided in either case such person is over 18 years old. The Company re- serves the right to refuse any Cardholder nomination by the Customer. 2.2. The Company may offer different types of Card Products from time to time or may restrict or limit the type of features of such products. Customers should consult on the relevant Card Product/features in the lists of fees and charges for Cards. The Company may make available additional infor- mation/descriptions on its Website or at Customer’s request. 2.3. Cards are delivered in a sealed envelope either to the Card- holder in person or by postal means to postal address spec- ified in Application, together with Card Instructions. 2.4. Customer acknowledges the risks related to Card delivery by postal services if Customer chooses or accepts such de- livery method, and accepts that The Company shall not be responsible or liable for risks, actions, delays or non-delivery by third party mail services. 2.5. Customer/Cardholder are obliged to inform The Company and Partner (as the case may be) if Card is not received by Cardholder within a reasonable timeframe by postal means. If Card is not received within 30 (thirty) days, The Company is entitled to cancel the Card, and issue a replacement Card upon Customer’s application which may be subject to fees and charges.
