Claims Reporting. Client shall make a first report to Client’s Insurer and PEO, on such form or forms as may be designated by said Insurer from time to time, within 24 hours of notice of the occurrence of any injury or accident which is work-related or which Client believes may be work-related. Client agrees to pay any additional costs (including fines, judgments, settlements or legal fees) caused by Client’s failure to timely report any such injury or accident. The final adjudication for workers’ compensation purposes whether the injury or accident is work-related shall be made by the PEO or its designated representative.
Claims Reporting. Notice of any claim must be provided to the Fund as required by law or the applicable Coverage Agreement. Failure by Fund Member to timely report a claim may result in denial of coverage or payment of fines or penalties imposed by law or regulatory agencies. If the Fund advances payment of any fine or penalty arising from Fund Member’s late claim reporting, Fund Member will reimburse the Fund for all such costs.
Claims Reporting. In the event that Tenant reports any claims to its insurers issuing the coverage required under this Lease, Tenant shall advise Landlord of the same. Tenant shall diligently pursue coverage of claims under all such insurance policies and keep Landlord advised of any claim denials, whether partial denials or full denials, and the basis for the same.

(v) a fatality, spinal cord injuries with paralysis, brain damage, severe burn injuries resulting in disfigurement or scarring, massive internal injuries affecting body organs (e.g., heart, lungs), multiple fractures, amputations, total or partial blindness in one or both eyes, nerve damage causing paralysis and loss of sensation in arm and hand, permanent disabilities, sexual molestation or abuse, or environmental related damage or injury.
Claims Reporting.
(a) At MMSM’s discretion, responsibility for claim reporting under Section 3.3.2(b) shall be assigned by the Contractor to the Designated Post-Collection Service Provider.
(b) All loads must be documented by Contractor or the Designated Post-Collection Service Provider, as the case may be, and include requirements defined in 3.3.1(c) and such other information as MMSM may designate (collectively, “Claim Information”). Claim Information with respect to any delivery of Mandatory Materials PPP to the Designated Post-Collection Facility must be submitted within 10 Business Days of the delivery date.
(c) MMSM will issue a claim summary to Contractor based on Claim Information directly provided to MMSM pursuant to Section 3.3.2(b), and Contractor will review the claim summary for accuracy. Contractor must report to MMSM any content in the claim summary that Contractor disputes within five days of the claim summary being issued.
(d) After MMSM has approved the Claim Information, MMSM will issue a purchase order to Contractor, including a reference number. MMSM may, at its discretion, choose to issue payment to Contractor based on the approved purchase order without the need for Contractor to submit an invoice. Where invoices are required by MMSM, Contractor will invoice MMSM using the contact information provided by MMSM for such purpose (as may be updated by MMSM from time to time).
(e) Standard tare weights for specific trucks may only be used on specific written permission of MMSM.
Claims Reporting. It is a condition precedent to Company’s liability that claims are reported as required in Section 21 below. In case of theft or other criminal act which may be the subject claim under this Agreement the Participant must give immediate notice to the South African Police Service and cooperate with the Company in securing the conviction of the offender.
