Division Responsibilities. 1.3.1 Provide maps of each property and forest management plans to the contractor to aid in reaching the Division’s forest resource management goals and objectives. Existing GIS information will also be available upon request.

Division Responsibilities. 1. The Division shall:
a. Ensure AdSS provides training to outpatient providers serving Members on conditional release and ensure outpatient providers demonstrate understanding of A.R.S. § 13-3991 and A.R.S. §§ 13-3994 through 13- 4000, duties of outpatient providers; and
b. Establish relationships with the Superior Court and ASH to support streamlined communication and collaboration between the Division, outpatient treatment team, ASH, and the Superior Court.
2. The Division Behavioral Health Complex Care Specialist shall coordinate with the Support Coordinator and care manager, as needed, for Members on Conditional Release from the Arizona State Hospital (ASH) consistent with the CRP issued by the Superior Court to facilitate discharge.
3. The Division shall monitor the status and outcomes of Members subject to a Conditional Release issued by the Superior Court and AHCCCS, including through the Conditional Release Monthly Monitoring Report as directed by the Contractor Chart of Deliverables
4. The Division shall follow all obligations, including those stated in this section, applicable to it as set forth as specified in A.R.S. § 13-3994.
Division Responsibilities. Division responsibilities and/or requirements in support of this Agreement include: Provide the services described in this SLA and Division product description (see page 8). Meet appropriate response times and associated deadlines. Comply with all statutes and rules as required by the Utah Procurement Code (63G-6) and Utah Administrative Code (R33) Ensure appropriate internal controls are in place for procurements, accounting operations, and contract reviews. Division Responsibilities. The Director of Technology shall serve as the administrator overseeing Division IT systems. The principal or department head shall serve as the site coordinator overseeing IT systems within a school or department/office. The Instructional and Administrative Technology Department or designee shall be responsible for, among other things, authorizing and establishing employee, non‐employee, and student IT accounts, setting quotas for disk usage on IT systems, establishing a retention schedule, establishing a Division virus protection process, providing safe access to Internet resources, and ensuring that processes are in place requiring each employee and non‐employee accessing the Division system to sign the Acceptable Use Policy Agreement (AUP) and for parents/legal guardians to acknowledge the receipt of and compliance with the AUP as included in the Standards for Student Conduct.

5.2 The DIVISION will notify VENDOR of changes made to the Delaware Medical Assistance program (DMAP) Provider Specific Policy for DDDS Lifespan Home and Community Based Waiver and State Plan Rehabilitative Services for Persons with Intellectual Developmental Disabilities, DDDS Policies and Procedures, DDDS Provider Standards for Home and Community Based Services or any other documents incorporated by reference.

Division Responsibilities. The DIVISION shall make available to the VENDOR the plan of care (ELP or other plan of care) for every client that chooses to be served by the VENDOR. The DIVISION shall assign a Case Manager to each waiver client and non-waiver clients receiving residential services to monitor the implementation of the program plan, coordinate the provision of services required by the plan, monitor the quality and quantity of services provided, and evaluate the progress of each client. The DIVISION will assign a Family Support Specialist to coordinate care for non-waiver, non-residential clients. DDDS will notify VENDOR of any changes made to the Delaware Medical Assistance Program (DMAP) DDDS HCBS Waiver Provider Specific manual or any other documents incorporated by reference. In the event that a client seeks to change providers or services, the DIVISION will assist the client in finding a suitable alternative.
Division Responsibilities. The Division will provide person centered planning referrals to the Qualified Vendor. The support coordinator will attend the person centered planning session and assist in identifying location and times. The support coordinator will coordinate follow up on action steps identified in the person centered plan.
Division Responsibilities a. Bilateral Visual Impairment: DBS has the primary responsibility of serving any eligible individual who has bilateral visual impairment which with the best correction, for that individual, constitutes or results in a substantial impediment to employment. Such an individual may have other disabilities which are in addition to the bilateral visual impairment, such as other physical and/or mental disabilities. Such an individual can be considered a dual client and can be served by both divisions; both divisions may simultaneously maintain an open case on such an individual. DBS and DVR are to coordinate the provision of the services under an Individualized Plan for Employment (IPE) for the person. The IPE will identify the services provided and/or coordinated by the most appropriate division and also the division which is responsible for payment of the services.
