Doctor's Certification Clause Samples
Doctor's Certification. The Employer may require a doctor's certification to substantiate sick leave usage should the employee be absent from work in excess of three days or if the Employer has good reason to suspect sick leave abuse.
Doctor's Certification. When a teacher has been absent for more than four (4) consecutive days, a doctor's certificate, certifying personal illness or illness in the immediate family, must be submitted to the building principal.
Doctor's Certification. Except in situations covered by FMLA, in which case the documentation and certification and recertification requirements would apply, the Department Head and/or Human Resources Manager may require under certain circumstances (to include but not limited to excessive sick leave usage, identifiable patterns of sick leave, unauthorized sick leave) a physician’s certification as to the duration of the time period requiring absence from work to substantiate the approval of sick leave. The authentication may be by certificate from a licensed physician, licensed chiropractor or other license professional, or such means as may otherwise be determined by the City.

Doctor's Certification. When claiming compensation for sick leave, the employee may establish to the satisfaction of their Department Head the authenticity of illness of the employee, death of an immediate family member or illness of an immediate member of the family. The authentication may be by certificate from a licensed physician, licensed chiropractor or other licensed professional, or such means as may otherwise be determined by the City.
Doctor's Certification. Employees may be required to furnish a doctor's certificate after three (3) consecutive days, or more, of absence to substantiate the approval of sick leave, at the employer's expense. A physical examination may be required by a physician before return to work, at the employer's expense. Only basic work days shall be paid and charged against sick leave.
Doctor's Certification. Sick leave with pay in excess of three (3) consecutive working days or two (2) shifts for reasons of personal illness or physical incapacity shall be granted only after presentation of a written statement by a licensed physician to the employee's immediate supervisor certifying that the employee's condition prevented the employee from performing the duties of the employee's position and that the employee is able to return to work. In the case of sickness arising or relating in any way to pregnancy, use of sick leave is limited to approval signified by presentation of a written statement from a licensed physician.

They City may, through the employee, request verification from the employee’s health care provider that substantiates the injury or illness preventing the employee from working. An employee may be required by their department head or designee to provide a release to return to work form the employee’s health care provider upon injury or illness.
