Goal Statement. The Contractor shall continue the model of care integration and Member Engagement (as described in Section 6) to improve the rate at which Covered Individuals who had been hospitalized for a mental illness receive timely and adequate mental health services aftercare using Behavioral Health Covered Services and reduction in the rate of re-hospitalization. The technical specifications for measures one and two (M1 and M2), including the denominator and numerator definitions, shall conform to the HEDIS FUH specifications published by NCQA, and shall be applied to the measurement period corresponding to the current Contract Year. The contractor shall report performance as per the 2018 HEDIS measure specification. Within M3(A), the definition of Readmission shall mean the number of episode discharges from a mental health 24-hour Level of Care and the number of discharges that were followed by a subsequent admission to the same or equivalent 24-hour Level of Care within 0-90 days of the discharge date of the episode. Using the count of discharges and the count of readmissions, the rate of readmission is calculated. Within M4, the definition of “arranged prior to discharge” shall mean as evidenced within either the MHS/Connect (the Contractor’s clinical documentation application) or the Provider’s medical record, documentation of: Member and/or family involvement and agreement, the name of the Behavioral Health Covered Service, name of the Provider, and the date and time of the first appointment M1. the rate for 7-day follow-up per HEDIS 2018 specifications greater than or equal to the HEDIS 2017 50th percentile $50,000 greater than or equal to the HEDIS 2017 75th percentile $75,000 M2. the rate for 30-day follow-up per HEDIS 2018 specifications greater than or equal to the HEDIS 2017 50th percentile $50,000 greater than or equal to the HEDIS 2017 75th percentile $75,000 M3(A). Goal Statement. The Progress Foundation Seniors Program consists of ▇▇▇▇▇▇▇ House and Rypins House, which are Transitional Residential Treatment Programs (TRTPs). The goals of the program are: To maximize individuals' efforts to achieve the highest possible level of self-sufficiency by continuing the rehabilitation process begun in acute and sub-acute residential programs. To divert as many persons as possible from institutional placements, such as skilled nursing facilities, and "L" facilities, by providing an alternative setting. To reduce recidivism by providing a therapeutic setting in which individuals can grow toward independent living by emphasizing the acquisition and application of survival skills. The development of personal support systems and placement of as many clients as possible in educational, volunteer and vocational or pre-vocational training situations, as well as in jobs in preparation for more independent living.
Goal Statement. What is the goal of your program and how does it align with the scope of work described in the RFP guidelines? The program outlined in this proposal addresses Goal 2 (The district and every school in it is a place where children, staff, and families thrive) and Goal 3 (African-American children and youth excel in school) as described in the MMSD’s 2018 Strategic Framework. Since 2017, Urban Triage (UT) has built and delivered programs that ▇▇▇▇▇▇, develop, and strengthen vulnerable populations, centering Black students, youth, and families in areas of economic empowerment, self-sufficiency, community leadership, advocacy, and family success through transformative education, community engagement, trauma response, healing, and cultural heritage. The activities described below are informed by best practices and empirical research leading to positive learning and socio-emotional outcomes for the populations served in this grant. Our program will achieve these goals by developing school-program partnerships, re-formatting UT curriculum and programs (e.g., Supporting Healthy Black Youth 2020, Thoreau Elementary pilot 2022), and engaging young adults in activities such as homework help, academic support, sports, artistic experiences, and other activities that are known to help young adults develop life skills, form positive relationships with adults, and ease the transition to adulthood. The program also addresses areas identified as critical needs for high school students in the 2023 Child Youth Policy Paper. Specifically, wellness activities/social-emotional programming, arts and music, and leadership opportunities are needed.
