Annual Eligibility Certification (12/5/16)19 PFIs and Servicers must submit a complete and accurate Annual Eligibility Certification (AEC) certifying and affirming compliance with all of the provisions and requirements of the Applicable Agreements and the Guides as a condition of continuing eligibility. The AEC consists of the following documentation: • Annual Eligibility Certification form (Form OG1); • MPF Fidelity and Errors & Omissions Insurance Worksheet (Form OG2); • Any supporting documentation required to be submitted with Form OG1 or Form OG2; • Any action plan required to remediate non-compliance with any MPF Program Requirement; and • Any other documentation requested by the MPF Bank or MPF Provider. Student’s residence rights under this Agreement may be lost due to failure to meet academic requirements, the imposition of disciplinary sanctions, or cancellation of the Agreement by the University after the student’s breach of the Agreement. If a student is academically suspended, his/her housing agreement is immediately cancelled. If the suspended student submits an academically based appeal to the Academic Standards Committee in care of the Registrar’s Office; (within eight working days of the date of the notice of dismissal) and the appeal is granted by the Academic Standards Committee, the student is then eligible for University housing. No refund of housing charges (including deposit) will be made to students suspended or dismissed for disciplinary action or breach of this Agreement (see paragraph W). However, in all events, the University reserves the right to exclude immediately, without prior notice, any student whose behavior presents a substantial risk to the safety or health of self or other students, or whose behavior presents a reasonable likelihood of imminent substantial disruption of normal residence activities.

Assume in all of the following Examples that a weekly Employee has qualified for paid illness or injury leave under the Employment Standards Act by aggregating consecutive days worked on Seasons 1 and 2 of an episodic television series as described in Article 23.1(c) (and as illustrated in Examples 1 and 3 above), and that the Employee continues working as a weekly Employee until the end of Season 2. If the Employee then returns at the beginning of Season 4 of the same series and works as a weekly Employee until the end of Season 4, the Employee likewise remains eligible for paid illness or injury leave until the end of Season 4. In this Example, the Employee’s eligibility for paid illness or injury leave expires at the end of Season 2, because the Employee does not return at the beginning of Season 3. In order to requalify for paid illness or injury leave, the Employee must again satisfy the ninety (90) consecutive day requirement, beginning with the first day of employment on Season 3. In this Example, the Employee remains eligible for paid illness or injury leave until the date of resignation, termination or layoff in Season 3, at which point the Employee’s eligibility expires. When the Employee returns at the beginning of Season 4, the Employee must again satisfy the ninety (90) consecutive day requirement in order to requalify for paid illness or injury leave.
