Certification Form. (a) Except as otherwise specified as contemplated by Section 301 for Securities of any series, whenever any certification is required to be given pursuant to Section 305(d)(i) of this Indenture in connection with the transfer of a beneficial interest in a Restricted Global Security to a person who wishes to take delivery thereof in the form of a beneficial interest in a Regulation S Global Security, such certification shall be provided substantially in the form of Annex A to this Indenture, with only such changes as shall be approved in writing by the Issuer. (b) Except as otherwise specified as contemplated by Section 301 for Securities of any series, whenever any certification is required to be given pursuant to Section 305(d)(ii) of this Indenture in connection with the transfer of a beneficial interest in a Restricted Global Security to a person who wishes to take delivery thereof in the form of a beneficial interest in an Unrestricted Global Security, such certification shall be provided substantially in the form of Annex B to this Indenture, with only such changes as shall be approved in writing by the Issuer. (c) Except as otherwise specified as contemplated by Section 301 for Securities of any series, whenever any certifications are required to be given pursuant to Section 305(d)(iii) of this Indenture in connection with the transfer of a beneficial interest in the Regulation S Global Security to a person who wishes to take delivery thereof in the form of a beneficial interest in the Restricted Global Security, such certifications shall be provided substantially in the form of Annex C to this Indenture, with only such changes as shall be approved in writing by the Issuer. (d) Except as otherwise specified as contemplated by Section 301 for Securities of any series, whenever any certification is required to be given pursuant to Section 305(d)(iv) of this Indenture in connection with the transfer of a beneficial interest in an Unrestricted Global Security to a person who wishes to take delivery thereof in the form of a beneficial interest in the Restricted Global Security, such certification shall be provided substantially in the form of Annex D to this Indenture, with only such changes as shall be approved in writing by the Issuer.

Certification Form. Unit members shall be required to sign the MiraCosta Online Class Requirements Self- Checklist, which is attached as Exhibit G. By signing the certification form, unit members agree to abide by all recommendations on the checklist. Those who do so are agreeing to meet these recommendations in each class taught in an online or hybrid format on an ongoing basis. Unit members shall be required to recertify once every four (4) years to maintain currency to teach online. The appropriate ▇▇▇▇ or designee shall document that all unit members who are assigned to teach distance education courses have satisfied the certification requirements above. The ▇▇▇▇ or designee shall maintain a comprehensive list of certified online instructors, and shall consult this list prior to assigning a unit member to teach a distance education course in accordance with department and district procedures. The district must inform unit members when recertification is required at the beginning of the academic year that the certification will expire. If a recertification notice is not provided on time, the unit member will maintain certification until notice is given and the unit member is afforded three (3) months to complete the process. Exceptions to this requirement may be granted by the appropriate vice president, such as in emergency situations that require late instructor assignments. Efforts will be made to provide appropriate distance education training to the unit member during the semester of the assignment.

Certification Form. The field representative shall execute a certificate in the format shown in GSFIC Forms Packet, on the letterhead of the manufacturer, certifying that "the equipment has been installed in strict compliance with the recommendations of the manufacturer and is operating properly.” All certification forms shall be provided to Design Professional at least seven (7) days prior to the inspection for Material Completion. Initial Test and Balance. Prior to Material Completion, CM/GC shall perform an initial test and balance on the heating, ventilating and air conditioning system. CM/GC shall submit the initial test and balance report to Design Professional at least seven (7) days prior to the proposed date of Inspection for Material Completion. Two (2) additional Test and Balance Reports are required after Material Completion and occupancy.

SkyBridge requires all candidates to acknowledge the information given is to the best of their knowledge. Any discrepancies result in either dismal of candidates or termination or contractor.  How the Respondent will conduct interviews and include interview criteria. o SkyBridge implements a multitier interview process.  1st – phone screen to review information from both the candidate and potential project. Answer any questions or concerns and make sure the potential candidate meets minimum qualification for the position.  2nd – Face-to-face interview. If an in-person interview cannot be completed, a web-conference (ex. Skype, face-time) will be completed. During this interview a more in-depth review of abilities, resume details, and opportunity will be reviewed and verified.  As per individual Client request, either a verbal retelling of the candidate’s qualification or a fully documented interview criteria will be provided to all SkyBridge’s Clients.  How the Respondent will conduct reference checks on staff. o References will be obtained during the interview process and completed internally. Documentation is always completed and given upon request.
