Participant Agrees to. Comply with the Program participation requirements and obligations set out in the Program Guide.
Participant Agrees to. (a) Comply with all terms and conditions of this Agreement.
(b) Submit the following information to SBCTA, through the System, for each vanpool passenger before such passenger rides with the vanpool: name, email address, phone number, address, employer name and phone number.
(c) Upon SBCTA’s approval of the Participant, Participant agrees to receiving emails as they relate to the SB Loop program. If at any time, the Participant wishes to unsubscribe from the participation–related notifications, they will have to terminate their participation in the SB Loop program.
(d) Participant understands that the remaining lease amount not covered by SBCTA’s subsidy is billed by the Vendor to the vanpool Leaseholder, who may recover the balance of costs from the vanpool passengers. Should vanpool passengers leave the vanpool, the Leaseholder and the remaining vanpool passengers are responsible for covering all non-subsidy vanpool costs.
(e) The Leaseholder is responsible for maintaining the application information submitted into ▇▇▇▇▇▇▇▇▇▇▇.▇▇▇. Any changes in your contact information, employer information, passenger list, route, drop off or pickup locations, or scheduling information, shall be updated no later than five (5) business days after the change takes place.
(f) Should in the future you have changes to your vanpool’s Primary Driver, Back Up Driver(s) or Reporter(s), those persons will be required to submit an online Participation Agreement.

Participant Agrees to. FOREVER RELEASE AND TO NOT ▇▇▇ SKI APACHE, its directors, employees, volunteers, agents or representatives for any injuries or property damage caused by or resulting from any access to Ski Apache’s premises or by participation in any winter activities, including but not limited to incidents arising out of Ski Apache’s alleged negligence. Participant further agrees for him/herself and his/her heirs, assigns and representatives to indemnify and hold harmless Ski Apache from any and all losses, claims or proceedings of any kind which may be initiated by Participant or by any other person or organization on Participant’s behalf, including but not limited to reasonable attorney fees incurred by Ski Apache.

c. Upon Metro's approval of the Participant, Participant agrees to receiving emails as they relate to the Metro Vanpool program. If at any time, the Participant wishes to unsubscribe from the participation–related notifications, they will have to terminate their participation in the Metro Vanpool program.
d. Participant understands that the remaining lease amount not covered by Metro's subsidy is billed by the Vendor to the vanpool.
e. The Coordinator is responsible for maintaining the application information submitted into Metro Vanpool. Any changes in your contact information, employer information, passenger list, route, drop off or pickup locations, or scheduling information, shall be updated no later than five (5) business days after the change takes place.
f. Should in the future you have changes to your vanpool's Primary Driver or Reporter(s), those persons will be required to submit an online Participation Agreement.
Participant Agrees to a) Provide the PCQN with timely, accurate care process and outcome data on palliative care services as such data are outlined and specified by the PCQN. The palliative care processes and outcome data will be collected concurrently.
b) Send two team members to each PCQN conference.
c) Submit data on an ongoing basis to the PCQN data center (“PCQN Data Center”) no later than 30 days past the discharge date for the patient. Data are to be submitted as specified in instructions provided by the PCQN Data Center.

e) Include only the individual patient identifiers listed in Attachment II, attached hereto and incorporated by reference, in data submitted for inclusion in the PCQN Data base. PCQN shall aggregate the data that it receives from each Participant into the PCQN data set (the “PCQN Data Set”).
f) Designate a contact person for the PCQN, in writing, who will serve as a liaison between the Participant and PCQN for quality improvement and data collection activities.
g) Supply own hardware (i.e., computer) and software necessary for data entry.
h) Designate appropriate personnel (e.g., clinical or administrative support staff) to receive training (either in person or by written materials) and, using a form provided by PCQN, or equivalent, test Participant personnel responsible for data collection and data entry on understanding of and adherence to specified protocols to assure a minimum standard of data quality. If site personnel require additional training such training will be provided by the PCQN.
i) Develop and maintain an interdisciplinary committee to participate in performance improvement activities with consultation and assistance of the PCQN.
j) Allow the PCQN use (including analysis, sharing and publication) of aggregated data for analysis. In providing data to the PCQN, Participant understands that it shall be the only member of the PCQN to view its individual data disaggregated from the PCQN database. No other member of the PCQN, nor PCQN itself, shall be able to use the individual data separate from the aggregate report.
k) Notify PCQN immediately of any personnel changes relevant to access to the PCQN Database, particularly the departure of a PCQN Database user. Upon notification by Participant, PCQN staff will deactivate access for all personnel no longer employed by the Participant.
Participant Agrees to. FOREVER RELEASE AND TO NOT SUE SUMMIT, its directors, employees, volunteers, agents, or representatives for any injuries or property damage caused by or resulting from any access to Resorts’ premises or by participation in any winter activities, INCLUDING BUT NOT LIMITED TO INCIDENTS ARISING OUT OF SUMMIT’S ALLEGED NEGLIGENCE. Participant further agrees for himself/herself and his/her heirs, assigns and representatives to INDEMNIFY AND HOLD HARMLESS SUMMIT from any and all losses, claims, or proceedings of any kind which may be initiated by Participant or by any other person or organization on Participant’s behalf, including but not limited to reasonable attorney fees incurred by SUMMIT.
Participant Agrees to. Collect, concurrently with delivery of care to patients, health care process and outcomes data regarding maternal and infant health and the provision of maternity care services, and provide such data in a timely and accurate manner. Specifically, Participant agrees to disclose to the Oregon-MDC the data elements (“Participant Data”) identified in Attachment A, which constitute a Limited Data Set, as defined in 45 C.F.R. § 164.514(e)(2).
Participant Agrees to. Attend a participant orientation offered by the Service Provider and understand the goals and expectations of the work experience opportunity.
