Reports and Forms. WorkSafeBC owns all right, title and interest in the reports and forms used in the performance of the Services under this Agreement and no such right, title and interest shall vest in the Contractor as a result of its use (or that of its Personnel) of such reports and forms.
Reports and Forms a) EVERTEC will produce and send to BPPR the reports identified in the Operating Rules and/or Service Riders, as applicable. The frequency of the reports will also be specified in the Operating Rules and/or Service Riders, as applicable. BPPR will be responsible for promptly reviewing and reconciling the reports, statements and files, and any notice, correspondence or communication it receives from EVERTEC. Should BPPR identify any omission or discrepancy between its records and the data provided by EVERTEC, or if it has an objection to information in any report, it must notify EVERTEC in writing within ten (10) days following the receipt of the report. EVERTEC will process the investigation according to the procedures set forth in the Operating Rules. BPPR agrees and acknowledges that its failure to report the omission, discrepancy or objection within the time set forth in this Section 2.8 will release EVERTEC from any liability regarding such omission, discrepancy or objection. b) Should a form be needed in conjunction with the Services, BPPR will use one provided or accepted by EVERTEC. All information provided to EVERTEC by BPPR must be complete and legible. EVERTEC may, but is not required to, communicate with BPPR in order to verify the incomplete or illegible information and will not be held responsible for any errors in rendering the Services due to incomplete or illegible information provided by BPPR.
Reports and Forms. During the term of this Agreement, the Servicer shall promptly and routinely furnish to FMC, sorted by SPE Owner, and to each Owner copies of all material reports, records, and other documents and data as required by the Servicing Guidelines or as otherwise required by this Agreement, including the reports set forth on the Required Reports Schedule. All monthly reports shall be delivered in accordance with (a) the data security requirements of the Secure Communication Protocol made a part of the Program Manual, and (b) the Required Reports Schedule, unless otherwise expressly provided for herein. In the event that any reports are not delivered to FMC as provided for herein, all fees due and owing the Servicer under this Agreement may be withheld until such reports are received by FMC. During the discovery process a framework will be established for the design and development of all necessary reports that have been identified. Contractor will be responsible for providing a Report and Form Development Strategy, as well as consulting support for the County report activities during the report

Reports and Forms. 2.1 Within five (5) business days of receiving a request from T-Mobile for any report or form filed, or prepared for filing, by Provider with the Federal Communications Commission (“FCC”) or any other governmental authority that relates to the Services, Provider shall provide T-Mobile with the requested report or form, including, but not limited to: (a) FCC Forms 477; (b) FCC Forms 499-A; (c) FCC Forms 499-Q; (d) FCC Forms 502; (e) International traffic and revenue reports; (f) Network Outage Reporting System (“NORS”) reports; and (g) Rural Call Completion (“RCC”) reports. 2.2 Provider shall provide T-Mobile with a draft of any report or form that Provider intends to file with the FCC or any other governmental authority as soon as possible but no later than five (5) business days before the intended filing date (unless the deadline for filing is shorter than five (5) business days) if the filing of such draft report might reasonably be interpreted as evidence that T-Mobile has violated applicable Law. 2.3 If Provider has not filed, or does not intend to file, any report or form that T-Mobile has requested and Provider is obligated to provide pursuant to this Section 2, Provider shall provide T-Mobile with a written explanation for Provider’s decision not to file the requested report or form as soon as reasonably possible but no later than five (5) days after receipt by Provider of T-Mobile’s request for the report or form.
Reports and Forms. 5.10.8.1 SYSTEM MUST PROVIDE INITIAL BOOKING FORMS AND REPORTS DESIGNED FOR COUNTY NEEDS. THE BIDDER MUST SEND SAMPLES OF STANDARD REPORTS, AUDIT TRAIL REPORTS, MANAGEMENT REPORTS AND FORMS WITH THE BID. THE ACTUAL FORMS AND REPORTS SHALL BE DESIGNED WITH THE CONTRACTOR. System WILL provide initial booking forms and reports designed for County needs. XImage Corporation WILL DEVELOP samples of standard reports, audit trail reports, management reports and forms to the specifications of the County. Additional form design will be negotiated upon contract negotiation. 5.10.8.2 MUST BE ABLE TO CREATE REPORTS USING THE AUDIT LOG DATABASE. The system WILL be able to create reports using the audit log database. 5.10.8.3 FORM AND REPORT DESIGN MUST INCLUDE CAPABILITIES SUCH AS BOXES, COLUMNS, ROWS, MULTIPLE FONTS, FORM FEED, PAGE BREAKS, LOGO, LOCATION AND SIZE OF IMAGE VARYING FROM 1" X 1" to 8.5" X 11", ETC. Form and report design WILL include capabilities such as boxes, columns, rows, multiple fonts, form feed, page breaks, logo, location and size of image varying from 1" X 1" to 8.5" X 11", etc. 5.10.8.4 SYSTEM MUST ALLOW THE SYSTEM ADMINISTRATOR TO DESIGN FORMS AND REPORTS. BIDDER MUST DESCRIBE THE PROCESS (PROGRAMMING LANGUAGES AND TOOLS) THAT WOULD ALLOW THE SYSTEM ADMINISTRATOR TO DESIGN FORMS AND REPORTS. System WILL allow the System Administrator to design forms and reports. Reports and Forms. 4.4.1 All inspection reports and other documentation shall be on the appropriate reporting forms provided by KDA, as directed by the KDA Division of Food Safety and Lodging. 4.4.2 All inspection reports and other required documentation shall be submitted to the KDA on a weekly basis. 4.4.3 The LOCAL AGENCY shall submit completed inspection reports and other documentation to KDA electronically using the inspection software provided to the LOCAL AGENCY by KDA pursuant to section 4.5.
Reports and Forms. Agent shall prepare and submit in a manner and time satisfactory to Owner and Authority all forms and reports required by Owner and Authority. Such reports and forms shall include, but not be limited to: (a) the Plan; (b) a monthly occupancy report; (c) a monthly statement outlining the expenditures of Agent on behalf of Owner incurred during the previous month; and (d) a weekly report to the management agent for the Development, Owner and Authority regarding deposits. Agent agrees that representatives of Owner and Authority shall have full and free access, during working hours or on reasonable advance request, to all such records and books of account of the Development, including the right to make photostatic copies of, or excerpts of transcriptions from, such records and books of account and related supporting documents and statements (including but not limited to bank statements, checks paid by banks and check stubs).
