Required Components. At a minimum, the screening must include questions that enable the HMO to identify the following: a) The member’s chronic physical, mental and behavioral health illness(es) (e.g. respiratory disease, cardiac disease, stroke, diabetes/pre-diabetes, renal disease, back pain and musculoskeletal disorders, cancer, overweight/obesity, all mental health and substance abuse disorders). b) The member’s perception of their strengths, their general well- being (including chronic conditions and access to prescription medications). c) If the member has a usual source of care. d) Any indirect supports the member may have (family and social supports). e) Any relationships the member may have with community resources. f) Any immediate and/or long-term concerns a member may have about their overall well-being (e.g. social determinants of health). g) If the member needs assistance to conduct activities of daily living (including but not limited to bathing, dressing and eating) as well as instrumental activities of daily living (including but not limited to medication management, money management and transportation).
Required Components a) The Contractor shall impose obligations and duties on its subcontractors, including its Participating Providers, that are consistent with this Agreement, and that do not impair any rights accorded to LDSS, SDOH, or DHHS. b) No subcontract, including any Provider Agreement shall limit or terminate the Contractor's duties and obligations under this Agreement. c) Nothing contained in this Agreement between LDSS and the Contractor shall create any contractual relationship between any subcontractor of the Contractor, including Participating Providers, and the County or LDSS.

Identification of health conditions that require prompt medical attention such as acute illness, chronic disease(s) requiring immediate medical management and/or treatment (e.g. asthma, diabetes, seizure disorder), signs of infection or communicable disease, nutritional problems, pregnancy, and significant developmental or mental health conditions. ii. Unclothed, symptom-targeted physical examination, including injury surveillance. iii. Identification of medical treatment and/or follow up that may be required prior to the comprehensive initial health assessment which is completed within 30 days of entering out-of-home care.

SECTION 22 (PROVIDER AGREEMENTS) October 1, 2004 d) Any subcontract entered into by the Contractor shall fulfill the requirements of 42 CFR Part 434 that are appropriate to the service or activity delegated under such subcontract. e) The Contractor shall also ensure that, in the event the Contractor fails to pay any subcontractor, including any Participating Provider in accordance with the subcontract or Provider Agreement, the subcontractor or Participating Provider will not seek payment from the LDSS, the Enrollees, or their eligible dependents. f) The Contractor shall include in every Provider Agreement a procedure for the resolution of disputes between the Contractor and its Participating Providers. g) The Contractor shall ensure that all Provider Agreements entered into with Providers require acceptance of a woman's enrollment in the MCO as sufficient to provide services to her newborn, unless the newborn is excluded from participating in Medicaid managed care.
Required Components. The Tiered Trigger Level Framework (the “Trigger Framework”) shall be comprised of the following types of Trigger Levels: Instream Surface Water Trigger Levels, Trigger Levels for Direct Discharges, and Trigger Levels for Indirect Discharges. Each type of Trigger Levels shall have three tiers unless the relevant Oversight Committee decides otherwise.

The activities and reporting responsibilities delegated to the subcontractor; and provide for revoking the delegation, in whole or in part, and imposing other sanctions if the subcontractor’s performance is inadequate, and an obligation for the provider to take corrective action; 2. That the subcontractor specifically agrees to be bound by confidentiality provisions; 4. The monitoring oversight of subcontractor performance by the contractor; 5. Provisions allowing DMAHS and DHHS to evaluate through inspection or other means, the quality, appropriateness and timeliness of services performed under a subcontract to provide medical services (42 C.F.R. § 434.6(a)(5)); 6. Provisions pertaining to the maintenance of an appropriate record system for services to enrollees. (42 C.F.R. § 434.6(a)(7)); 7. Sufficient provisions to safeguard all rights of enrollees and to ensure that the subcontract complies with all applicable State and federal laws, including confidentiality and non-discrimination. B. The Contractor shall impose obligations and duties on its subcontractors, including its Participating Providers, that are consistent with this contract, and that do not impair any rights accorded to DHS or DHHS.
