SHORT FORM OF EVALUATION. 1. All employees who contract again with the District after their first three (3) years of service in the Newport School District shall be defined as continuing employees and may be evaluated pursuant to this Article. 2. After a beginning employee has four (4) consecutive years of satisfactory evaluations, he/she shall have the option of using the short form of evaluation. 3. Employees who choose the short form evaluation shall notify their principal/supervisor by October 1. Notice will be placed in their file accordingly. 4. Continuing employees who choose the short process shall have one (1) thirty (30) minute required observation before May 30. The principal will sign off on the evaluation form that the employee has given satisfactory service. The short form of evaluation may not be used as a basis for determining that an employee’s work is unsatisfactory or as probable cause for non-renewal of an employee’s contract. 5. If the evaluator or employee has reason to believe the short process option should be dropped during the year, written notification must be given of the reasons for the decision by February 1. If they disagree, the principal shall have the right to the final decision. The employee shall have the right to appeal to the Superintendent. 6. If a teacher changes grade level configurations (P, K-4, 5-8, 9-12), the teacher will remain on his/her current form of evaluation.

After an employee has three (3) consecutive years of satisfactory evaluations under provisions of this Agreement, the principal or other evaluator may elect to use a short form of evaluation (see Appendix C2 or C3). The short form of evaluation shall consist of either a thirty (30) minute observation during the school year with a written summary or a final annual written evaluation based on the evaluation criteria and based on at least two (2) observation periods during the school year totaling at least sixty (60) minutes without a written summary of such observations being prepared. B. However, the normal evaluation process set forth in this Article, Sections 4 and 5, shall be followed at least once every three (3) years for each employee. In the event that the Short Form Summary indicates an overall determination of unsatisfactory, the employee will be provided a mutually developed plan of improvement to be followed for the remainder of the contract year and the normal evaluation process will be used for the ensuing contract year. An employee may request that the normal evaluation process be conducted in any given school year. C. The short form evaluation process may not be used as a basis for determining that an employee's work is unsatisfactory or as probable cause for non-renewal of the employee's contract.

After a regular employee has four (4) consecutive years of satisfactory evaluations under provisions of this Agreement, the principal or other evaluator and the affected employee must mutually agree to use a short form of evaluation. (see Appendix B-8) The principal or other supervisor shall notify each selected employee on or before October 1 that the employee may be evaluated by the short form. B. The short form of evaluation shall consist of either a thirty (30) minute observation during the school year with a written summary or a final annual written evaluation based on the employee evaluation criteria and based on at least two (2) observation periods during the school year totaling at least sixty (60) minutes without a written summary of such observations being prepared, with post observation conferences. C. The normal evaluation process set forth in the preceding provisions of Article IV, Section 5.1-5.3 shall be followed at least once every three (3) years for each employee, and an employee or principal or other supervisor may request that the normal evaluation process be conducted in any given school year. D. Short form evaluations shall be completed on or before May 1. The short form evaluation process may not be used as the basis for determining that an employee’s work is unsatisfactory nor as probable cause for non-renewal of the employee’s contract.

  • Form of Agreement If a vendor submitting an Proposal requires TIPS and/or TIPS Member to sign an additional agreement, a copy of the proposed agreement must be included with the proposal. In response to submitted supplemental Vendor Agreement documents, TIPS will review proposed vendor Agreement documents. Supplemental Vendor’s Agreement documents shall not become part of TIPS’s Agreement with vendor unless and until an authorized representative of TIPS reviews and approves it.

The Local Sponsor must submit work products directly to the appropriate state or federal regulatory agencies in accordance with permit conditions to be eligible for reimbursement under this task, unless otherwise directed.

  • System Description The wet detention basin is designed to trap 80% of sediment in runoff and maintain pre-development downstream peak flows. The basin has two forebays (smaller ponds) located at the low end of two grass ▇▇▇▇▇▇. In addition to runoff conveyance, the grass ▇▇▇▇▇▇ also allow infiltration and filtering of pollutants, especially from smaller storms. The forebays are each 4 feet deep. They are connected to the main pool by 18 and 24-inch metal pipes that outlet onto a rock chute. The forebays will trap coarse sediments in runoff, such as road sands, thus reducing maintenance of the main basin. The main pool will trap the finer suspended sediment. To do this, the pond size, water level and outlet structures must be maintained as specified in this Agreement (see Figures 1, 2 and 3). The main basin receives runoff from a 67.1 acre drainage area (41.2 acres within the subdivision and 25.9 acres off- site drainage coming from the east). During high rainfall or snow melt events, the water level will temporarily rise and slowly drain down to the elevation of the control structure. The water level is controlled by a 12-inch concrete pipe extending through the berm in the northwest corner of the basin (see Figures 1 and 3). On the face of the 12- inch pipe, there is metal plate with a 3-inch drilled hole (orifice) with stone in front of it. This orifice controls the water level and causes the pond to temporarily rise during runoff events. Washed stone (1-2” diameter) is placed in front of the orifice to prevent clogging. High flows may enter the grated concrete riser or flow over the rock lined emergency spillway. “As-built” construction drawings of the basin, showing actual dimensions, elevations, outlet structures, etc. will be recorded as an addendum(s) to this agreement within 60 days after Waukesha County accepts verification of construction from the project engineer.
