The "Child and Family" clause defines the rights and responsibilities of parties regarding children and family matters within the context of the agreement. It typically addresses issues such as custody arrangements, visitation schedules, parental decision-making authority, and the welfare of any children involved. By clearly outlining these aspects, the clause helps prevent disputes and ensures that the best interests of the child are prioritized, providing structure and predictability for all parties.
Child and Family. Team (CFT): Formerly known as Team Decision 17 Making (TDM) is a group process facilitated by Social Services Agency (SSA) 18 wherein individuals are convened and engaged through a variety of team-based 19 processes to identify the strengths and needs of the child or youth and his or 20 her family, to help achieve positive outcomes for safety, permanency and well- 21 being.
Child and Family. Care leaves are not considered breaks in service for purpose of eligibility for employee benefits, longevity, or seniority under this Agreement.
Child and Family. Teams (CFT)
40. Defendants will institute contract language by October 2012 requiring PIHPs to develop Child and Family Teams (CFT) consistent with the T.R. Principles and the Core Practice Model. Class members, especially those who are served by multiple child-serving agencies, whose individualized strengths and needs (as identified by a standardized assessment process) indicate that they would benefit by having a Child and Family Team should be provided an organized Child and Family Team. In such cases the CFT drives the treatment planning process for services and supports. The role of the CFT is expressly set forth at Appendix E.
41. Defendants will establish a process for CFTs to obtain authorization by PIHPs. The process will be defined no later than November 1, 2012, whereby an organized CFT, with the participation of the Duly Authorized Provider Representative (DAPR), has the ability to access services and supports included in the class member’s treatment plan.
42. In case of any disagreement between the DAPR and other CFT members regarding service recommendations, the DAPR works with the CFT to resolve any differences of opinion, with particular attention to the input and preferences of the parent(s)/guardian(s) of the enrolled child. When Agreement is not able to be reached regarding service planning at the CFT level, the DAPR helps unite the team in agreeing to try a particular service approach on an interim basis for a specified period of time during which the behavioral outcomes of the child are carefully monitored by the CFT. The team later reconvenes to consider the outcomes in relation to the services that have been provided and works together to make needed adjustments as time progresses. When deciding upon a service approach in the case of disagreement between CFT members, the DAPR gives as much or more weight to the opinions and preferences of the parent/guardian/youth as to any other member of the CFT.
43. Defendants will establish Agreements across the Department of Social and Health Services (DSHS) with child serving agencies requiring that local and regional representatives participate in CFTs (or care planning teams) for children served by multiple agencies.
44. Defendants will develop plans to provide guidance, training, supervision, and/or support to Child and Family Teams and informal care planning teams consistent with T.R. Principles and the Core Practice Model.
Child and Family. Centered Care Plan (CFCP) - The individualized, child-centered, strength-based, and family-focused plan of services and supports developed by the child and family team (CFT), the care management entity (CME), the OhioRISE Plan, or a combination thereof. Child and Family Team (CFT) - A group of people including the OhioRISE member and their family/caregiver(s), natural supports (relatives, friends, neighbors, etc.), and formal helpers (teachers, therapists, other professionals, etc.) who are involved with the child and family and who play an Claim – A bill from a provider for health care services assigned a unique identifier. A claim does not include an encounter form. A claim can include any of the following: (1) a bill for services; (2) a line item of services; or (3) all services for one member within a bill. Clean Claim – A claim that can be processed without obtaining additional information from the provider of a service or from a third party. Clean claims do not include payments made to a provider of service or a third party where the timing of the payment is not directly related to submission of a completed claim by the provider of service or third party (e.g., capitation). A clean claim also does not include a claim from a provider who is under investigation for fraud or abuse, or a claim under review for medical necessity. Consumer Contact Record – As defined in OAC rule 5160-26-01, the record containing demographic health-related information provided by an eligible individual, member, or the Ohio Department of Medicaid (ODM) that is used by the Ohio Medicaid consumer hotline to process membership transactions. Control Charts – A type of statistical process control tool that uses the relationship of observations to the mean and control limits to study how a process changes over time, also known as Shewhart charts. Covered Entity – A health plan, a health care clearinghouse, or health care provider under 45 CFR 160.103.
Child and Family. Team (CFT) Meetings. Upon request by County, Contractor shall attend CFT meetings. Attendance shall be in-person or by teleconference. County shall only pay Contractor when County specifically requests Contractor’s attendance, either in person or by teleconference. Multiple Units of Service shall be allowed upon approval of appropriate HHSA staff. $100 per one hour session Court Appearances. Upon Court subpoena and prorated for time actually spent at the pertinent court session. If Court Appearance is cancelled with less than 24 hours’ notice, Contractor may invoice for 2 hours of time. If Court Appearance is cancelled with 24 or more hours’ notice, Contractor may not invoice for the appearance. Child and Family. Team is a group of individuals responsible for the development, implementation, and monitoring of a unified IPoC that engages and involves the Child and family. The Child and Family Team is composed of family members, significant people in the lives of the Child and family, and representatives of the community’s human services agencies that can provide needed services.
