Health Management Program. A. The City and Local 145 agree to support a Health Management Program. Effective July 1, 1991, Local 145 will be allowed to provide input into the design of the Program prior to its implementation. Local 145 input must be received by Fire Management prior to September 1 of each fiscal year. The selection of the provider will be in accordance with Council Policy 300-7. B. During the term of this agreement, the City shall continue to fund the Overall Wellness Program up to $35,000 per year. This fund will be utilized for the Wellness Program, or the maintenance or purchase of weight machines and/or similar equipment by mutual agreement of the parties. C. During the term of this agreement, the City shall provide Hepatitis B vaccinations to all members of the unit.

The parties agree to administer the Wellness Program under the following terms: 1. Both parties agree that the Wellness Program is non-punitive. Participation is optional for employees who were hired as of November 2004. Participation is mandatory for employees hired after November 2004. The Program shall provide all five (5) of the following aspects of the Wellness Program: a. Medical b. Fitness c. Medical/Fitness/Injury Rehabilitation d. Behavioral Health e. Data Collection and Reporting The on-duty exercise program is mandatory and subject to disciplinary action due to non-compliance. 2. Employees who volunteer to participate in the Wellness Program will be required to take part in the entire program. If an employee is not participating in the comprehensive medical testing, they will still go to the Wellness Center for their respiratory fit clearance and DMV exams. 3. Employees who volunteer to participate in the Wellness Program have the option to withdraw at any time for any reason. 4. Employees will be scheduled for the physical examinations and fitness tests during off work time, for which they will be compensated. 5. Medical information collected during the physical examination and fitness testing process will be confidential. Medical records will be retained by the Program's medical provider and the specific details of the examination results will only be shared with the employee. Aggregate and non-confidential physical examination and fitness data will be provided to Local 145 and the IAFF National Database in accordance with the Wellness initiative. Health Management Program. PBM agrees to provide Health Management Programs to Covered Persons at United HealthCare's option. Health Management Programs developed during the term of this Agreement by PBM shall be made available to United HealthCare. ***
Health Management Program. 45. Health Management Program Defined. The SoonerCare Health Management Program (HMP) is offered statewide and serves SoonerCare Choice beneficiaries ages 4 through 63 with chronic illness who are at highest risk for adverse outcomes and increased health care expenditures. HMP beneficiaries are selected using HMP predictive modeling software. The state must include in the Quarterly Operational Report, described in paragraph 52, a report on HMP activities including a description of populations served and services provided.

The SoonerCare Health Management Program (SHMP) is offered to SoonerCare Choice beneficiaries statewide, who are at highest risk for adverse outcomes and increased health care expenditures. The program serves beneficiaries ages 4 through 63 with a chronic illness, and are selected by the SHMP using predictive modeling software. The State must include in the Quarterly Report, described in paragraph 47, a report on SHMP activities including a description of populations served and services provided.
Health Management Program. PBM agrees to provide Health Management Programs to Covered Persons at United HealthCare's option. Health Management Programs developed during the term of this Agreement by PBM shall be made available to United HealthCare. * At United HealthCare's reasonable request, PBM will work with United HealthCare to obtain incremental funding from pharmaceutical companies for such Health Management Programs as mutually agreed. * If United HealthCare requests assistance to develop its own health management program, PBM shall assist United HealthCare in obtaining external funding for development. At United HealthCare's option and if PBM is willing, PBM shall develop with United HealthCare and shall share revenues with United HealthCare on any programs involving health management, outcome research, clinical trials development and post market surveillance. Nothing in this section requires United HealthCare to utilize PBM's Health Management Program or prohibits United HealthCare from developing or seeking funding for health management programs and other related programs outside of PBM and this Agreement.

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.
