Identification Required Clause Samples

Identification Required. Any Board employee who is required to submit a sample for drug/alcohol testing to the designated collection facility may be required to show proof of identity which must include a picture I.D. to the personnel at the designated collection facility before providing the sample.

Identification Required. Contractor's field operations personnel, who normally come into direct contact with the public, including drivers, shall bear some means of individual photographic identification such as a name tag or identification card. Upon request by the Customer or County, the Contractor’s employee will display his or her employee identification card.

Identification Required. Company shall provide its employees, Affiliates and subcontractors with identification for all individuals who may make personal contact with residents or businesses in City. City may require Company to notify Customers yearly of the form of that identification. Company shall provide a list of current employees, Affiliates, and subcontractors to City upon request. City reserves the right to perform a security and identification check through law enforcement agencies upon Company and all its present and future employees, in accordance with accepted procedures established by City, or for probable cause.

Identification Required. Grantee shall provide every employee, contractor, grantees, or sub-grantees who is in contact with the public with identification cards and badges. Upon request of City, Grantee shall notify all Customers of the form of identification that each employee shall be carrying or displaying so that all Customers may easily identify one of Grantee's employees. The Grantee shall provide City with a list of current employees, contractors, grantees, and sub-contractors to City upon request.

Identification Required. No financial institution shall effect any transaction with respect to which a re­ port is required unless the person or organizations with whom such trans­ action is to be effected has been satis­ factorily identified.
