Monitoring & Evaluation. The Provider will: 8.1. Undertake a half termly review of each referred/commissioned students’ academic and personal performance, which can involve parents/carers. 8.2. Undertake a more formal termly review meeting to which Commissioners, parents/carers and all other professionals will be invited. 8.3. Supply the Commissioner with a formal termly report on all referred students 8.4. For full time students an annual report will be issued to parents at the end of the school year. 8.5. Allow and positively encourage Commissioners to undertake monitoring visits to the Provider’s main site and 3rd party commissioned provision on a pre-arranged basis. 8.6. Hold a monitoring and evaluation meeting with the Commissioner on at least a termly basis.
Monitoring & Evaluation. It is a requirement of the service that appropriate records are kept and maintained by the pharmacy contractor, to enable verification of service provision and training undertaken by staff. These records may be used for information for internal and external audit and evaluation purposes. For IEP the recording system is currently neo360®.
Monitoring & Evaluation. 6.1 The submission of ‘Patient Compliance Reporting Forms’ (Appendix 1) and content within can be used for the purposes of auditing SLA compliance. 6.2 Information pertaining to all patients and claims should be retained in the pharmacy and be made available to NHS Grampian, if requested, for audit purposes.

Under the agreement pharmacy contractors will be required to confirm that they have complied with any requirements concerning the sourcing of medicines supplied. Payment verification will be used to validate claims for individual contractors. Contractors will be required to maintain procurement records for supplies made under this agreement for 2 years following supply for the purposes of payment verification. 8.2 Tiers 2 and 3 will require bespoke monitoring and audit with appropriate KPIs reflecting the care bundles contracted for in addition to the requirements relating to Tier 1. 8.3 Where a pharmacy has supplied a medicine from a source not specified or approved in the individual service pack for each medicine the Health Board retains the right to reclaim the appropriate service fee and make a claim against the contractor for the loss of primary care rebate.

Monitoring & Evaluation. Monitoring and Evaluation Wing of Pakistan Bait-ul-Mal shall strictly monitor the programme, through periodic visits of the district through out the year to assess efficacy of the project. Impact evaluation of the programme shall also be carried-out after one year of its launching to measure developmental out comes, suggest remedial measures so as to remove bottlenecks and bring improvement. All Directors of Provincial / Regional Offices should sent monthly progress reports on their performance to M&E Branch Head Office, who shall write them with their problems (if any)

This will be done by use of the Turas Vaccine Management Tool (The App). 7.2 A standard operating procedure(s) should be in place in the pharmacy to cover all aspects of service provision.
