Reporting and Analysis. (1) Provide regulatory (Securities and Exchange Commission), shareholder and other miscellaneous reporting and, in particular, prepare and maintain the following required books, records, and other documents:
(a) journals containing daily itemized records of all Portfolio securities purchases and sales, receipts and deliveries of securities, receipts and disbursements of cash, and all other debits and credits, in the form required by Rule 31a-1(b)(1) under the Act;
(b) general and auxiliary ledgers reflecting all asset, liability, reserve, capital, income and expense accounts, in the form required by Rules 31a-1(b)(1)(i) - (iii) under the Act;
(c) a securities record or ledger reflecting separately for each portfolio security as of trade date all “long” and “short” positions, if any, carried by the Portfolios for the accounts of the Portfolios, and showing the location of all securities long and the off-setting positions of all securities short, in the form required by Rule 31a-1(b)(3) under the Act;
(d) a record of all Portfolio purchases or sales, in the form required by Rule 31a-1(b)(6) under the Act;
(e) a record of all puts, calls, spreads, straddles and other options, if any, in which the Portfolios have any direct or indirect interest or which the Portfolios have granted or guaranteed, in the form required by Rule 31a-1(b)(7) under the Act;
(f) a record of the proof of money balances in all ledger accounts maintained pursuant to this Agreement, in the form required by Rule 31a-1(b)(8) under the Act;
(g) price make-up sheets and such records as are necessary to reflect the determination of the net asset values of each class of the Portfolios;
(h) Regulatory: semi-annual and annual Form N-SARs and quarterly Form 13-Fs.
(i) Shareholder: semi-annual and annual statements of assets and liabilities, operations, changes in net assets, per share data, appreciation/depreciation, and share activity;
(j) Media: weekly, monthly, quarterly, semi-annual and annual statistical data of the Funds, to be provided to newsletters and other investment industry publications such as lCl, D▇▇▇▇▇▇, Lipper and the FINRA; and
(k) Board of Trustees: such financial or accounting reports regarding the Portfolios as may be requested by the Fund’s Board of Trustees from time to time. Reporting and Analysis. The Supplier shall provide reporting and analysis on all aspects of the Services being provided to the Customer, including:
a) provide Management Information (MI) detailing work carried out by the Supplier on the basis of the billable charged hours as outlined in Section 4 (How Services will be bought (Call for Competition Process))
b) related reporting as specified, to include measures on effectiveness and efficiency (these shall be outlined in each Project Specification)
c) reporting to demonstrate progress and effectiveness (as defined by the Customer).
Reporting and Analysis. Responsible for the timely and accurate reporting and analysis, both written and oral, of financial and associated business information to internal, external, and regulatory constituencies.
Reporting and Analysis. The Agency shall provide reporting and analysis on all aspects of the Services being provided to the Client, including:
a) provide Management Information detailing work carried out by the Agency on the basis of the billable charged hours as outlined in Section 3 (How Services will be bought (Call-Off Process))
b) campaign related reporting as specified, to include measures on effectiveness and efficiency (these shall be outlined in each Brief)
c) reporting to demonstrate progress and effectiveness (as defined by the Client).
Reporting and Analysis. 3.8.1. Contractor shall maintain and update a HIMIS which allows Users to produce, print, and/or download reports in .PDF, MS Word, MS Excel, .CSV, or .TXT format.
3.8.2. Contractor shall compare data from multiple health plans in a single report format.
3.8.3. Contractor shall provide a means for analysis using any data field available.
3.8.4. Contractor shall provide a wide range of reports that are pre-defined, such as clinical-based reports, financial reports, eligibility reports, and pharmacy reports.
3.8.5. Contractor shall produce cost trend reports on pharmacy, inpatient and outpatient hospital, professional, or surgical claims/experience, in total, per case, per Member (per capita), and/or per Enrollee.
3.8.6. Contractor shall produce utilization trend reports on admissions or services (medical and pharmacy), length of stay, days of care, number, and setting (inpatient/outpatient/physician office) of surgical cases.
3.8.7. Contractor shall produce reports on overall frequency of inpatient and outpatient surgeries, appropriateness of site of care, and total cost of both inpatient and outpatient surgeries.
3.8.8. Contractor shall produce evaluation reports on cost-of-benefits, discount savings, and claims lag time with normative comparisons.
3.8.9. Contractor shall support International Statistical Classification of Diseases ICD-9 and ICD-10 claims data.
3.8.10. Contractor shall produce clinical evaluation reports with comparison on:
3.8.10.1. Use and cost by Major Diagnostic Category (MDC) and Diagnosis Related Group (DRG);
3.8.10.2. Use and cost by Episode Group;
3.8.10.3. Reports to compare cost effectiveness of inpatient, outpatient, and physician office surgeries; and
3.8.10.4. Member health risk.
3.8.11. Contractor shall provide quality of care reports which include:
3.8.11.1. Information on outliers, re-admissions, complications of treatment, nosocomial infections, and deaths, by provider, in total, and per case; and
3.8.11.2. Agency for Healthcare Research and Quality (AHRQ) quality indicators or related indicators and HEDIS measurements, or similar quality measurement standards.
3.8.12. Contractor shall have clinical reports for the following areas: 3.8.12.1. Disease management 3.8.12.2. Gaps in care 3.8.12.3. Population health 3.8.12.4. Preventive care 3.8.12.5. Reporting and Analysis. The Mandatory Medicaid and Children’s Health Insurance Program (CHIP) Core Set Reporting final rule released on August 31, 2023. To meet this mandatory reporting requirement, states must report on all the measures on the Child Core Set and the behavioral health measures on the Adult Core Set by December 31, 2024, for FFY 2024 state reporting and annually thereafter. This data provides information for CMS and states to better identify quality improvement priorities and to plan and implement quality improvement initiatives. DVHA completes this required Core Set reporting each year by the deadline set by CMS. BreakPoint Labs personnel will provide a comprehensive report(s) that provides detailed information for every vulnerability, expressly articulates the business risk associated with each vulnerability, and provide recommendations for remediation actions. Topic areas will include, as a minimum:
1. A detailed description of the service provider’s approach and methodology process (including tools used, scripts followed, date and time that each test was performed).
2. The test description and corresponding results – all identified vulnerabilities must be clearly stated and risk ranked.
3. Implications of the test results, including risk factors.
4. Recommendations for resolving negative test results.
5. Summary of informational objects that can be accessed from sources external to the network.
6. Regardless of the test result, recommendations for further improvements and/or implementations for best practice strategies. The analysis process employed by BPL takes into account and ensures an understanding of the impact vulnerabilities can have for the specific business or organization. Organizations often put more focus on confidentiality, whereas others focus more heavily on the availability of their systems. BPLs analysis process also considers how vulnerabilities can relate; such as combining several low and informational findings to expose a critical risk to the organization.
