Service Timeline Expectations The Enrollment and Orientation period is intended to take place 1-60 days from the referral date, to include the following required actions and deadlines: Welcome Call 3 business days from referral date IRIS Consultant Selection 3 business days from welcome call IRIS Consultant Auto-assign 4 business days from welcome call Initial Enrollment & Orientation Visit 14 calendar days from referral date ISSP Plan – Completed 30 calendar days from initial visit Implementation of ISSP Plan – Completed and Approved 45 calendar days from initial visit Transition Visit On or before 55 - 60 calendar days from welcome call Transition to participant- selected level of IRIS Consultant involvement On or before 61 calendar days from referral date Date of annual plan submission/approval Prior to current plan year end ICAs are responsible for enrolling participants in the program as well as providing orientation services. Orientation includes an introduction to the participant’s role and responsibility in the IRIS program and development of the Individualized Support and Services Plan (ISSP). Welcome Call & IRIS Consultant Selection The ICA will enter the referral into the Department case management system (WISITS) within one business day. The ICA will contact the individual within three business days after receiving the participant’s referral packet to assist the participant in choosing a consultant. The ICA should obtain information from the participant about special needs or preferences and, in turn, provide the participant with at least three available consultants who are most suited to meet the participant’s specified needs and provide IRIS consultant biographies, in the DHS required format. If the participant prefers to have a consultant chosen on his or her behalf, then the participant is assigned an IRIS consultant during this welcome call based on information provided by the participant to the ICA. Voluntary Enrollment. The District will ensure, and incorporate into the Plan specific criteria and procedures to ensure, that the parents or guardians of all newcomer students are consistently notified that enrollment at the Newcomer Academy is voluntary. The District will adequately and timely notify parents that they may enroll their child in either their home schools or the Newcomer Academy (that language assistance services are offered in the home schools and that placement in the Newcomer Academy is an option). The notice will detail the benefits and possible detriments of both placements and will be provided in the language and medium of communication needed by the parents. The District will document these communications (e.g., maintain a log or list of use of translated notices and interpreters).
Voluntary Enrollment. Enrollment in a Commercial Fleet Vehicle Program is voluntary and may be terminated at any time by either party, for any reason. Voluntary Enrollment. A Member who voluntarily seeks assistance for a substance abuse or an alcohol problem, before being required to submit to a reasonable suspicion test or a random test may be tested pursuant to Section 14.5, and shall not be subject to discipline if the Member completes the recommended course of treatment as required by the counseling agency or physician. Failure to cooperate with or complete the treatment program shall result in discipline up to and including suspension and such Member shall also comply with Section 14.8 (A) thru (F). If the Member is ordered to undergo testing for alcohol or illegal drug use after a voluntary request for assistance, the Member shall be placed on paid administrative leave until such time as the testing is completed and the course of treatment has been prescribed. The Member who has requested assistance prior to a requirement for testing shall then be placed on his earned paid leave pending completion of the prescribed course of treatment whenever such treatment conflicts with the Member’s duty days. If paid leave is exhausted, the City shall grant an unpaid leave of absence or family medical leave pending completion of the prescribed course of treatment. A voluntary request for assistance will not preclude any follow-up test requirements or disciplinary action subsequent to the voluntary enrollment as indicated herein. To qualify for this Voluntary Enrollment, a Member must notify the Fire Chief of his need for assistance prior to receiving any notification from the City that a drug/alcohol test will be required.
