Attendance Records. Attendance records (individual time sheets) shall be maintained for all personnel paid with CDBG funds that are involved in operating the Project. This applies to part-time as well as full-time personnel. In addition to the accounting for daily attendance, the type of leave taken (annual, sick or other), shall be disclosed. Daily attendance records must support budgetary charges for payroll purposes.
Attendance Records. A. CONTRACTOR shall keep daily attendance of each student and shall report attendance monthly to LEA. Such attendance shall be filed with monthly invoices to LEA within thirty (30) days of the close of the school month. Separate attendance register forms must be submitted for all related services as specified on the IEP.
B. Original attendance registers submitted to the LEA with invoices for payment must be completed by the service provider whose signature must appear on said register. CONTRACTOR is responsible for verifying accuracy of said registers and for informing service providers of their personal responsibility for the completion and accuracy of said attendance registers.
C. CONTRACTOR shall permit LEA representatives, upon reasonable notice, to meet with staff of CONTRACTOR for the purpose of auditing attendance reporting.
D. The LEA is required to maintain school records in order to ensure the appropriate high school graduation credit is received by the student. The contract may allow for partial or full-time attendance at the NPS.
Attendance Records. Employees shall be in attendance at work in accordance with the rules regarding hours of work, holidays, and leaves. The department shall keep daily attendance records of employees. Employees who are unable to report for work for any reason shall notify the department no later than one (1) hour prior to their scheduled starting time. The Fire Chief may waive this requirement upon presentation of a reasonable excuse by the employee.
Attendance Records. The Provider shall collect and report daily attendance information on each enrolled child in the following manner:
1. The Provider and District will work together to address chronic absenteeism which is defined as missing ten (10) percent of school days, or 18 days of a 180-day school year. Providers with excessive absenteeism rates should create an action plan.
2. If any enrolled child is absent without excuse for up to ten (10) consecutive days of school, the District and Provider shall make every effort to contact the family and get the child to school. The District and Provider may begin attempts to contact the family prior to ten (10) consecutive unexcused absences.
3. The provider will make a concerted attempt to get the child to come to school. If these attempts fail and the child is absent without excuse for ten (10) consecutive days of school, the child shall no longer be enrolled and the District/Provider may fill the slot with another child.
4. Daily attendance reports shall be provided to the District on a monthly basis.
5. In accordance with NJAC 6A:13A-4.4 (g), preschool children shall not be expelled or suspended, which includes sending children home early or excluding children for any reason other than medical such as injury or illness. Positive behavior supports through the Pyramid Model for Supporting Social Emotional Competencies shall be employed to reduce or eliminate challenging behaviors.
Attendance Records. MATC shall provide MPS with the attendance records of pupils attending MATC pursuant to this Agreement on a weekly basis. MATC shall implement an attendance policy as stipulated by the MATC Attendance Policy provided in Appendix B.
