Contents of File. The personnel file shall consist of (but not by way of limitation) the following: application for employment; letters of reference, other than those which are exempt from disclosure under law; employee performance evaluations; letters of recommendation, praise, or thanks; disciplinary material and letter of resignation.
Contents of File. 1. Each teacher’s personnel file shall contain the following items of information: a. All teacher evaluation reports b. A copy of the teaching certificate c. Transcript of academic records d. Tenure recommendation e. Copies of individual contracts f. Any other pertinent personnel material. 2. Any (negative) material placed in a teacher’s personnel file must be submitted to the teacher for his or her initials before it is placed in said file. The teacher shall be allowed the opportunity to examine and file a response said response shall become a part of this file. The Association President will be notified if the teacher refuses to initial this material.
Contents of File. A. The diocesan office file will contain the following: 1. current valid teaching certificate/license; 2. current valid catechist certificate--Diocese of Youngstown (original copy) *; 3. application form; 4. transcripts (official); 5. Authorization and Verification for 2008 child protection policy; 6. Acknowledgement of Receipt of child protection policy;

Contents of File. (a) No material shall be placed in the teacher’s personnel file unless such teacher has received a copy of said material. This material refers to any report or disciplinary letter produced by Board administration. The teacher shall initial the material, verifying receipt and retain a copy of such. (b) The Board reserves the right to retain such information in the personnel file as may reasonably be required to comply with the requirements of any federal or provincial statutes and regulations.
Contents of File. A. All items, including written evaluations, placed in the file shall be signed and dated by the person placing it in the file and a copy shall be given to the employee prior to placing it in the file. B. Anonymous letters or materials shall not be placed in a personnel file nor shall they be made a matter of record. C. Employees shall have the right to submit a written commentary/rebuttal to any material placed in the file, and such written commentary/rebuttal shall be attached to the item in the file. D. Information in the personnel file that is proven inaccurate will be removed from the file by the Superintendent.
Contents of File. Personnel files shall be maintained at the District office and shall be the only official file for each unit member. 11.1.1 Files other than the official personnel file may be maintained, however, no derogatory information contained in such a file shall be entered into the official personnel file until the unit member has been given an opportunity to respond to such information as outlined in Article 11. 11.1.1.1 The supervisor will discuss problems or complaints with the unit member at the time they occur or as soon as possible thereafter.
Contents of File. The personnel file shall contain all materials relevant to the bargaining unit member’s employment. Each employee shall have the right, upon request, to review the contents of this file exclusive of materials received prior to the date of his/her employment by the District and also to receive a copy (at employee expense) of any documents contained therein. An employee shall be entitled to have a representative of the Council to accompany him/her during such review. Each employee’s personnel file subject to review shall contain the following minimum items: > All employee evaluation reports > Photostatic copy of the employee’s certificate/license > Transcript of academic records > Contract status recommendation Copies of annual contracts of the employee shall be available for inspection by the employee in the office of the clerk. An employee’s immediate supervisor may maintain a building or working file that is not part of the personnel file. The contents of the working file shall be available to the immediate supervisor, the employee or the employee’s designee, but the file is otherwise considered a confidential file. Material transferred from this file to the personnel file will be done only after written notification to the employee.
Contents of File. No material shall be placed in the personnel file unless the teacher has had an opportunity to review the material. The teacher shall acknowledge that he/she has had the opportunity to review such material by affixing his/her signature thereon. The teacher's signature does not indicate agreement with the contents thereof. The teacher shall also have the right to submit a written answer to such material and his/her answer shall be reviewed by the Superintendent or his/her designee and attached to the file copy.
Contents of File. 8 1. Only materials in the official District personnel file (augmented by records 9 maintained by other governmental agencies) shall be used in any proceeding 10 affecting the unit member's employment status with the District. 12 2. The following items, even though maintained separately, shall be considered 13 as part of the official District personnel file: time rosters, attendance and payroll 14 records, appointment forms, TB records, history cards, salary records, 15 schedule and assignment files. The District may add similar categories of 16 routine payroll/personnel record keeping to this list where such records are 17 part of the normal business operations of the Payroll/Personnel Sections.
