Performance Monitoring Reports. 1.1. Subject to the provisions of this Schedule 27.2, BA shall provide to Covad performance monitoring reports ("Performance Monitoring Reports") for services and facilities provided by BA. Istation shall provide access to performance monitoring reports and will provide a report reflecting any unscheduled outages to the reading diagnostic system within 10 business days after the conclusion of the month in which the unscheduled outages, if any, occurred. The report shall include the following details: Event Date and Time; Event Description; Duration; and Remedy. Without limiting the generality of the foregoing, for all Critical Severity Events not resolved within designated time frames, Istation will use commercially reasonable efforts to provide NCDPI with appropriate additional resources as necessary for expedient problem resolution and timely follow-up. NCDPI also acknowledges and agrees that Istation’s ability to resolve any problems hereunder on a timely basis may be dependent upon NCDPI’s cooperation in assisting with identifying, reporting, and replicating functionality problems hereunder, and providing other cooperation as may be reasonably necessary. Additionally, NCDPI agrees that Istation and NCDPI shall work together in good faith to undertake a root cause analysis to ascertain the source of the issue, and further work together in good faith and offer reasonable cooperation in an attempt to resolve any such issues or disputes relating thereto.
Performance Monitoring Reports. 8.1 The Authorised Officer (AN Other Organisation), or a representative specified by the Authorised Officer, shall at AN Other Organisation's discretion, conduct performance monitoring review meetings with the Provider twice per year – dates to be agreed between the two parties involved. These visits will be minuted and may include structured discussions with the management committee (Manager and Directors), staff and users, observation of service delivery, examination of service monitoring records, documents and/or reports AN Other Organisation may monitor performance standards where rECOrd deals with enquiries on AN Other Organisation's behalf, but will advise rECOrd of the monitoring techniques to be employed.
Performance Monitoring Reports. 4.1 Within twenty (20) Business Days of the end of each Service Quarter, the Contractor shall provide a Performance Monitoring Report to the Client. The Performance Monitoring Report shall be in the format agreed between the Parties and shall contain, as a minimum, the following information in relation to each Task Order in respect of the Service Quarter just ended: 4.1.1 a scorecard summarising the Contractor’s level of performance in relation to each Performance Level and setting out the detail in relation to the underlying figures; 4.1.2 a summary of all Performance Failures that occurred during the Service Quarter; 4.1.3 which Performance Failures remain outstanding and progress in resolving them; 4.1.4 for any Performance Failure, the cause of the fault and any action being taken to reduce the likelihood of recurrence; 4.1.5 for any Repeat Failures, actions taken to resolve the underlying cause and prevent recurrence; 4.1.6 the number of Performance Failure Points awarded to each Performance Failure in that Service Quarter; 4.1.7 any Earn Back Points that the Contractor considers should be taken into account as part of ongoing performing monitoring; 4.1.8 a rolling total of the number of Performance Failure Points that have occurred; and 4.1.9 such other details as the Client may reasonably require from time to time. 4.2 The draft Performance Monitoring Report shall be reviewed and its contents agreed by the parties at the Performance Review Meeting which immediately follows the issue of such report. 4.3 The parties shall attend Performance Review Meetings in each Service Quarter (unless otherwise agreed). The Performance Review Meetings will be the forum for the review by the Contractor and the Client of the Performance Monitoring Reports. The Performance Review Meetings shall take place at a date and time agreed by the Contractor and Client in the Service Quarter following the Service Quarter to which the Performance Monitoring Report pertains and shall be fully minuted. 4.4 The Contractor shall provide to the Client such supporting documentation as the Client may reasonably require in order to verify the level of the performance by the Contractor and the calculations of the amount of Performance Failure Points for any specified period. 4.5 If the Contractor fails to provide any Performance Monitoring Report in accordance with this paragraph 4 then it shall receive a RED Score.

  • Performance Monitoring ‌ A. Performance Monitoring of Subrecipient by County, State of California and/or HUD shall consist of requested and/or required written reporting, as well as onsite monitoring by County, State of California or HUD representatives. B. County shall periodically evaluate Subrecipient’s progress in complying with the terms of this Contract. Subrecipient shall cooperate fully during such monitoring. County shall report the findings of each monitoring to Subrecipient. C. County shall monitor the performance of Subrecipient against the goals, outcomes, milestones and performance standards required herein. Substandard performance, as determined by County, will constitute non-compliance with this Contract for which County may immediately terminate the Contract. If action to correct such substandard performance is not taken by Subrecipient within the time period specified by County, payment(s) will be denied in accordance with the provisions contained in this Paragraph 47 of this Contract. D. HUD in accordance with 24 CFR Part 570 Subpart O, 570.902, will annually review the performance of County to determine whether County has carried out its Community Development Block Grant (CDBG) assisted activities in a timely manner and has significantly disbursed CDBG funds and met the mandated “1.5 ratio” threshold. Subrecipient is responsible to ensure timely drawdown of funds.
