Submission of Data Clause Samples

Medical Group shall cooperate with PacifiCare in submitting to the Secretary of Health and Human Services statistical data pertaining to Covered Services provided by Medical Group, enrollment and disenrollment data and any other reports the Secretary may reasonably require to carry out its functions under the Medicare + Choice program.
Submission of Data. Each time Participant submits any Data to DNS-OARC, Participant must designate what restrictions, if any, are to be placed on the Data. This includes whether such Data are Confidential or Non-Confidential, attributable or anonymous, whether redistributable or not, and so forth, as appropriate. Participant is solely responsible for designation of any applicable restrictions. In the absence of any such designated restrictions, Raw Data submitted shall be deemed to be Confidential. Participant may change its designation of restrictions from time to time by written notice to DNS-OARC of such change.
Submission of Data. 1. I agree to submit the following documents stated below in each respective period and any other documents required for the maintenance of the loan contract on the bank's request according to bank's standard contract of loan transaction Article 17 and Article 19.
1. Each quarter : Surtax returns, trial balance of totals and balances, statement of current debts, sales estimates to customers and of items 2. Each half-year : Half-year accounts report, surtax returns, trial balance of totals and balances, statement of current debts, sales estimates to customers and of items 3. Each year : Certified public accountant's inspection report (financial statement of settlements), related statement of settlements, corporation registration transcript, business registry, shareholders book, article of corporation, summary sheet of earned income tax collected, estimated statement of settlements (3 years), state of major business relations, copies each permits and technology permit related documents (KS, ISO, patent rights, etc), labor-management dispute statement, miscellaneous product descriptions, concentric trades reference data, etc.
Submission of Data. Subject to the validity of a Certification as described in Article 4 of the Agreement, Dialysis Center, its Designated Affiliates and Eligible Managed Centers must provide to Amgen the Data in a machine readable format acceptable to Amgen, (Excel; or text file that is tab delimited, comma delimited, colon delimited or space delimited including a line of column headers identifying the column contents and units, if applicable. The Data files shall contain record counts for each file contained in the data submission); provided, however, that Dialysis Center shall be required to submit such test results only for those dialysis patients whose test results are actually determined by laboratories owned and operated by Dialysis Center.
Submission of Data. Client shall forward to Ceridian the necessary Input Data to complete the Services, which Input Data shall be provided to Ceridian as specified in the Processing Schedule to allow Ceridian adequate time to complete required processes. Without limiting any other term in the Dayforce General Terms, Client shall be responsible for the accuracy and completeness of the Input Data submitted and for any errors in and with respect to Input Data. Ceridian reserves the right to charge Client a Fee (as set forth in the Order Form and/or Rate Sheet) for any late submission of Input Data. These Fees will be assessed on a quarterly basis. If Ceridian does not charge the Fee within thirty (30) days after the end of the quarter, Ceridian will not retroactively charge Client for previous quarters.

2. I agree to submission of the following documents on request of the bank for the purpose of the company's foreign currency risk state and maintenance conditions in evaluating credit information.
1. Foreign currency risk maintenance structure and maintenance regulations conditions
2. Foreign currency supply and application statement
3. Foreign currency derivative product trade statement
Submission of Data. Without prejudice to their obligations to keep data up-to-date, Registrar shall provide Operator with complete and accurate registration or renewal data. Data submission shall be made on an irrevocable, non-exclusive, non-transferable and royalty-fee basis, so as to allow the Registry Operator to use such data to provide Registry Services
Submission of Data. Each MCP must submit clinical performance measurement data as required by ODJFS that enables ODJFS to calculate standard measures. Refer to Appendix M “Performance Evaluation” for a more comprehensive description of the clinical performance measures. Each MCP must also submit clinical performance measurement data as required by ODJFS that uses standard measures as specified by ODJFS. MCPs are required to submit Health Employer Data Information Set (HEDIS) audited data for the following measures:
a. Comprehensive Diabetes Care
b. Child Immunization Status
c. Adolescent Immunization Status The measures must have received a “report” designation from the HEDIS certified auditor and must be specific to the Medicaid population. Data must be submitted annually and in an electronic format. Data will be used for MCP clinical performance monitoring and will be incorporated into comparative reports developed by the EQRO. This requirement will be phased in over a two-year period. MCPs that do not have HEDIS-audited measures during calendar year (CY) 2004 will have the data collected and audited as part of the EQRO process. All MCPs will be required to submit the HEDIS-audited measures for the contract period beginning July 1, 2004.

3. I shall include the statement of current debts in Clause 1 with debt status, large amount loan status, repayment plans and loan plans according to the bank, but may replace it with credit information document (CRT information, etc) affixed to the end of this contract. If any information is missing from the above document, it should be recorded in an empty space then submitted.
4. If any documents in Clauses 1 through 3 is false, forged, altered or is missing information on purpose, I shall be responsible for the debt according to Article 7 of the standard contract, and will not dispute if registered as having poor credit record.
Submission of Data. In order to satisfy the requirements of this Section 1.1 and subject to the validity of a Certification as described in Article 4 of this Agreement, Dialysis Center, its Designated Affiliates and Eligible Managed Centers shall submit the following information to Amgen (or its designated data collection vendor) on a [DELETED] basis by the [DELETED] of the following [DELETED] (or the next business day if such last day is not a business day) : (i) all [DELETED] for each dialysis patient and the date of each such test (including test results for patients from at least [DELETED] of its Designated Affiliates and Eligible Managed Centers); (ii) a consistent, unique, alpha-numeric case identifier for each patient (which shall be sufficient to consistently track an individual patient without constituting Individually Identifiable Health Information or enabling identification of any previously de-identified data); and (iii) the name, address and phone number of the physical location at which such patient received treatment; provided, however, that Dialysis Center shall be required to submit such test results only for those dialysis patients whose test results are actually determined by laboratories owned and operated by Dialysis Center. To the extent that a Certification (as such term is defined in Article 4 of the Agreement) is not valid for any period in which Dialysis Center is otherwise obligated, pursuant to this Section 1.1.3 of this Exhibit 3.1, to submit Data, Dialysis Center shall independently calculate the appropriate clinical outcomes in accordance with the terms of this Section 1.1.3 of this Exhibit 3.1 and deliver the results of such outcome calculations in writing to Amgen, which submission to Amgen shall: (i) be deemed to satisfy Dialysis Center’s obligations under this Section 1.1.3 of this Exhibit 3.1 related to delivery of Data; and (ii) to the extent the [DELETED] is met following any such calculation by Dialysis Center, entitle Dialysis Center to continue to earn and receive incentives and rebates available under this Exhibit 3.1, in each case until such time as a Certification is then valid and Dialysis Center may resume submission of Data to Amgen. Upon such Certification, Dialysis Center shall submit to Amgen all Data dating back to the date Dialysis Center stopped submitting Data to Amgen within thirty (30) days. As described in Article 4 of the Agreement, to the extent Dialysis Center conducts such calculations, the calculation of Dialysis ...
