PAST PERFORMANCE- Factor 2. 3.2.1 The Offeror shall provide three (3) past performance references, if available, that reflect recent and relevant experience performed within three (3) years of the solicitation closing date specified in this requirement. The Offeror shall include a discussion on how the references provided are relevant to the work being proposed. Past performance references shall demonstrate the ability to handle the scope and breadth of the tasking in the SOW. The Offeror shall include contract/task order number, contract/task order type, program name, total contract/task order cost, short description of work performed, and names and valid telephone numbers for the Procuring Contract Officer (PCO), Contracting Officer’s Representative (COR), and Government Program Manager (PM). Each past performance reference shall not exceed two (2) pages per reference, for a total of six (6) pages. These pages will not be counted against the proposal page count identified in Instructions to Offerors 1.0 (B) (1). For each reference, the offeror shall identify its role as either a prime contractor or a subcontractor on the referenced contract. If the offeror was a subcontractor on the reference, then the offeror shall provide an explanation of its role in contract performance as well as the dollar value of its subcontract. 3.2.2 Subcontractors performing more than twenty-five percent (25%) of the Total Evaluated Price (refer to section MEvaluation Factors” (3. 1)) for details as to how the Government will calculate Total Evaluated Price) shall also submit three (3) past performance references, if available, that reflect recent and relevant experience performed within the three (3) years of the solicitation closing date specified in the RFP and demonstrate the ability to handle the scope and breadth of the tasking in the SOW. PAST PERFORMANCE- Factor 2. The past performance evaluation will be accomplished by reviewing aspects of an offeror’s recent and relevant past performance. Offerors may provide information for up to three (3) previous contracts whose effort is/was recent and relevant to the effort required by this solicitation.  Recent is defined as a contract in-progress with at least 50% complete or completed within the last three (3) years. If the offeror has not had three (3) relevant projects where it served as the prime contractor within the last three (3) years, information on recent and relevant subcontracts may be submitted.  Relevant is defined as a contract or combination of contracts that is cumulatively and concurrently of similar scope, magnitude, and complexity to the requirements as set forth in this solicitation. The Government may verify past performance information. The Government may contact some or all of the references provided, as appropriate, and may collect information through questionnaires, telephone interviews and existing data sources to include, but not limited to the Past Performance Information Retrieval System (PPIRS). The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources, including sources outside of the Government. This past performance information will be used for the evaluation of past performance. If subcontractor contracts are submitted, the offer must clearly indicate the percentage of work the subcontractor(s) will perform under each task/category of effort throughout the course of the contract. If subcontractor past performance is provided as part of the three (3) contracts or efforts; the subcontractor past performance will be given weight relative to the amount of effort under the solicitation the subcontractor is proposed to perform. Therefore, the offeror’s proposal should clearly detail the aspects of the work in the solicitation the subcontractor is proposed to perform. Offerors may also provide references where they have performed as a subcontractor. If past performance as a subcontractor is provided as part of the three (3) contracts or efforts, it will be given weight relative to the amount of effort performed under that contract.   • Performance Factors (a) Each party will notify the other party of the existence of a Performance Factor, as soon as reasonably possible after the party becomes aware of the Performance Factor. The Notice will: describe the Performance Factor and its actual or anticipated impact; include a description of any action the party is undertaking, or plans to undertake, to remedy or mitigate the Performance Factor; indicate whether the party is requesting a meeting to discuss the Performance Factor; and address any other issue or matter the party wishes to raise with the other party. (b) The recipient party will provide a written acknowledgment of receipt of the Notice within 7 Days of the date on which the Notice was received (“Date of the Notice”). (c) Where a meeting has been requested under paragraph 7.2(a)(3), the parties agree to meet and discuss the Performance Factors within 14 Days of the Date of the Notice, in accordance with the provisions of section 7.3.

  • Ongoing Performance Measures The Department intends to use performance-reporting tools in order to measure the performance of Contractor(s). These tools will include the Contractor Performance Survey (Exhibit G), to be completed by Customers on a quarterly basis. Such measures will allow the Department to better track Vendor performance through the term of the Contract(s) and ensure that Contractor(s) consistently provide quality services to the State and its Customers. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MFMP or on the Department's website).

  • Past Performance The Government will evaluate the contractor's performance on the NETCENTS-2 Orders provided in Exhibit B, CDRL B001. The PCO will determine the quality of the work performed based on an integrated assessment of data obtained in the Contractor Performance Assessment Reporting Systems (CPARS) and information obtained from Defense Contract Management Agency (DCMA) channels, interviews with customers, program managers and/or contracting officers for NETCENTS-2 task orders. Based on the contractor performance records above, the PCO will determine if there is an expectation that the contractor will successfully perform the required efforts under the unrestricted NetOps and Infrastructure Solutions contract.

  • Performance Measurement The Uniform Guidance requires completion of OMB-approved standard information collection forms (the PPR). The form focuses on outcomes, as related to the Federal Award Performance Goals that awarding Federal agencies are required to detail in the Awards.
