Outreach Program. F.1. The Companies will develop an Outreach Program which will be fully implemented within six months after the Effective Date of this Agreement. The Outreach Program will have at least the features described in Sections F.2 and F.3 of this Agreement.
F.2. The Companies will send a notice to all existing insureds whose medical policies were issued by the Companies prior to August 1, 2005. The notice will be substantially similar to the sample attached to this Agreement as Attachment F and will be subject to approval by the Monitoring Regulators. The Companies will also maintain a website that is responsive to coverage questions as set forth below. The notice will be mailed no later than six months after the Effective Date and will include the following: • A toll free number, mailing address, and email address for insureds to access with any questions. The toll-free number, mail and email will be directed to Company personnel as described in Section F.3 of this agreement who will be able to provide detailed information about the insured’s specific plan of insurance. • A website address for the Companies. The website content related to this Outreach Program will be acceptable to both parties and will include the following:
o A “frequently asked questions” section that answers relevant coverage questions, and which will be subject to an initial review by the Monitoring Regulators. o General coverage descriptions which will provide information about any existing coverages provided by the Companies including, but not limited to, preventive care, outpatient care for illnesses and injuries, inpatient hospital care, and emergency care (including care in Urgent Care centers and emergency rooms, and ambulance coverage).
o A listing of the mailing address, email address and toll free number to contact the Company personnel described in Section F.3. of this agreement. o Information about how to pursue the claims appeal and grievance process and contact information.
F.3. The Companies will provide a toll-free telephone number that insureds may call to ask any questions about their existing coverage. The number will connect them to appropriate Company personnel who will: • Be trained to respond to these calls • Be able to walk the insured through his or her coverage using plain language to confirm the insured’s understanding of the coverage. • Explain insurance coverages under the insured’s existing plans. • Offer to send the insured a complete certificate of insurance....
Outreach Program a. The Health Plan shall have an outreach program designed to encourage Enrollees to seek Behavioral Health Services through the Health Plan when the Health Plan, or Providers, perceive a need for Behavioral Health Services. In addition, the outreach program, at a minimum, shall provide for the following:
(1) Make available, by mail or at the request of an Enrollee/provider (participating or non-participating), outreach program documentation that is written at the fourth (4th) grade reading level and written in the primary language spoken by the Enrollee;
(2) A program to identify and manage Enrollees who are homeless.
(3) A program, including referral and other resources, designed to assist PCP's in the identification, management and treatment of:
(a) Enrollees with severe and persistent mental illness;
(b) Children/Adolescents with severe emotional disturbances; and
(c) Enrollees with clinical depression.
Outreach Program. 12.1 For the purposes of the Outreach Program outlined in this Section 12, “Class Member” does not include Automotive Recyclers. For greater certainty, the Outreach Program is not available for and does not apply to Automotive Recyclers.
12.2 The purpose of the Outreach Program is to eliminate alleged continuing damage to Class Members’ property, ameliorate alleged emotional distress and alleged mental anguish, and maximize Recall Remedy completion rates for Takata PSAN Inflator Recalls via traditional and non-traditional outreach efforts, including by expanding those currently being used by Honda.
12.3 Honda shall oversee and bear all costs of the Outreach Program, which shall be administered by the Honda Administrator.
12.4 The Honda Administrator shall oversee and administer the Outreach Program with the goal of maximizing, to the extent reasonably practicable, completion of the Recall Remedy in Subject Vehicles for the Takata PSAN Inflator Recalls.
12.5 The Outreach Program applies to Takata PSAN Inflator Recalls that are covered by NODs that have already been submitted to Transport Canada. The Outreach Program shall also apply to any additional Takata PSAN Inflator Recalls covered by an NOD that is submitted to Transport Canada in the future.
12.6 Honda shall have no duty to offer the Recall Remedy or the Outreach Program other than in respect of the Takata PSAN Inflator Recalls referenced in Section 12.5 above.
12.7 Honda shall provide notifications under the Outreach Program as follows:
A. For Class Members with Subject Vehicles containing ALPHA Inflators:
i. continued monthly notification to the Class Member via posted letter to uncompleted active registrations using updated CCMTA vehicle registration contact information;
ii. monthly posted letter notification will continue until the earlier of: (1) December 31, 2019; or (2) Substantial Completion;
iii. additional notifications will be delivered to Class Members via social media, e- mail, and/or by postings on a Honda website through December 31, 2019 or Substantial Completion, whichever comes first.
B. For Class Members with Subject Vehicles containing BETA Inflators (including Goldwing Motorcycles):
i. following the delivery of an initial NOD to vehicle registrants, re-notification approximately every SIX (6) months for any ongoing Recall via posted letter and/or electronic (e-mail) letter to uncompleted active registrations using the best possible contact information available to Honda until the...
Outreach Program. 1. Outreach program Enrollee communications that are written at the fourth (4th) grade reading level;
2. Outreach program communications that are written the primary language spoken by the Enrollee;
3. A program designed to assist PCP's in the identification and management, including referral and other resources, to aid in the treatment of:
(a) Enrollees with severe and persistent mental illness;
(b) Children/Adolescents with severe emotional disturbances; and
(c) Enrollees with clinical depression.
4. A program to identify and manage Enrollees who are homeless.
Outreach Program. The Trustees shall develop a program in which they affirmatively reach out to significant stockholders, the Board of Directors of the Company, management, key employees and representatives of each of the communities in which significant facilities are located to obtain input and insight from each of those constituencies regarding the matters that are significant to them, including, without limitation: (i) producing profitable and sustainable growth in the Company’s business and financial performance, (ii) building long-term stockholder value, (iii) following best practices in corporate governance, executive compensation, legal compliance, ethical conduct and corporate citizenship, and (iv) maintaining a strong and positive corporate culture.
Outreach Program. The Health Plan shall have an outreach program including but not limited to referral, training, consultation and other behavioral health resources designed to assist PCPs and other non-behavioral health providers in the identification, management and treatment of:
a. Enrollees with severe and persistent mental illness;
b. Children/adolescents with severe emotional disturbances; and
c. Enrollees with clinical depression.
Outreach Program. Under the Community Benefits Agreement, Developer has implemented (and is required thereunder to continue to implement or cause to be implemented) an Outreach Program. The Outreach Program is to facilitate the dissemination of important information, deadlines and other important information regarding the programs described in this Community Benefits Plan and in the Community Benefits Agreement to BVHP Area Businesses and residents. Vertical Developer shall from time to time, including upon request therefor from Developer or the Agency, provide information to Developer, the Agency and BVHP Area Businesses regarding deadlines and updates for the Project for inclusion in the Outreach Program. A list of applicable BVHP Area Businesses is maintained by and available from the Agency or Developer.
Outreach Program is committed to outreach to the community through leveraging her multidisciplinary expertise in several plasma physics/engineering and computational science areas. Tours at the CMUXE labs and demonstration to prospective students current research capabilities in areas of nuclear materials detection and monitoring, new materials design, photon sources for nanotechnology in actual and virtual reality domains will show the opportunities for multiple career paths and for the advancement of nuclear energy and related systems. This will help guiding and attracting high school students to the top current areas of studies and research. Indiana high school students will visit CMUXE labs where numerical and experimental simulations of powerful laser systems and intense particle beams will be demonstrated using various visualization technique and movies showing, e.g., the entire process of laser produced plasma development in experiments and in modeling at the same time. Graduate and undergraduate students will be involved in these demonstrations sharing their experience in the research. is planning also to give seminars for underrepresented minority students at Tuskegee University. is supervising currently one student from this university who applied to NE at Purdue after participation in SURF and NSF PIRE programs conducted at CMUXE center.
Outreach Program. It is the policy of the Department to provide Minority Business Enterprises (MBEs), Women Business Enterprises (WBEs) and all other business enterprises an equal opportunity to participate in the performance of all Department contracts. The Contractor shall assist the Department in implementing this policy and shall use its best effort to attain MBE and WBE participation of 15 percent and 7 percent, respectively, and to ensure that all available business enterprises, including MBEs and WBEs, have an equal opportunity to compete for and participate in the work of this agreement.
Outreach Program. The Northern and Isolation Travel Assistance Outreach Program (the “NITAOP”) is a two-component program consisting of the Northern and Isolation Travel Assistance Program and the Physician Outreach Program, that makes funding available to provide approved physicians with a travel time honorarium and reimbursement of travel expenses, for approved travel to the communities listed in Appendix B for the purpose of providing medical services to the residents of such communities, in accordance with the specific terms, conditions, rules and eligibility criteria approved or established by the JSC from time to time.
