RESPONSIBILITIES OF STUDENT. 4.1 Student acknowledges and agrees that they are subject to the terms and conditions contain in the Employment Contract and Student is therefore subject to normal management directions of Host Organisation. 4.2 In addition to Student’s obligations under the Employment Contract, Student acknowledges and agrees that: a) Student will have passed relevant competencies/subjects that will allow Student to operate at their assumed level of training; b) Student will be responsible, at their cost, for obtaining any criminal check, working with children check or other statutory check as required by Host Organisation prior to Student commencing the Placement; c) Student must satisfy all Occupational Health and Safety requirements and any other lawful orders, instructions, directions and requests of Host Organisation as advised by Host Organisation prior to Student commencing the Program or prior to commencing a particular task or regarding Client care; d) Student must negotiate and agree to the start and finish dates for the Program with Host Organisation; and e) Student must maintain confidentiality of all Confidential Information and Personal Information (including any Client information which is disclosed to Student in connection with the Program) in accordance with the Employment Contract and clauses 10 and 11 and must comply with the Host Organisation’s policies, procedures and reasonable directions in relation to Client confidentiality.
RESPONSIBILITIES OF STUDENT. To achieve the purposes stated above, STUDENT will, insofar as its means and the laws of the United States and the Republic of Italy will allow, be responsible for: 1. Meeting the academic and administrative prerequisites to attend the programs at the INSTITUTE; 2. Obtaining the required study VISA for Italy – STUDENT FAILURE TO OBTAIN A VISA FOR STUDY IS NOT GROUNDS FOR AVOIDANCE OF CANCELLATION FEES; 3. Securing privately, or through the HOME INSTITUTION, health insurance or the equivalent thereof, and repatriation insurance for the duration of their time studying abroad; 4. Reading all pre-program information provided by the INSTITUTE; 5. Paying all fees and other INSTITUTE invoiced costs on or before scheduled deadline dates; 6. Providing the INSTITUTE in advance of specified deadlines, a completed required application form along with course selections, housing preferences, special needs, etc.; 7. Respecting and adhering to the INSTITUTE’s academic policies and standards of conduct as well as relevant student codes of conduct and/or policies of academic integrity of the HOME INSTITUTION while studying at the INSTITUTE; 8. Maintaining communication, as needed, with parents, their families and the HOME INSTITUTION as allowed by FERPA (Family Educational Rights and Privacy Act), to assure prompt and satisfactory resolution of any and all problems that may arise; 9. Reviewing, acknowledging, and abiding by all INSTITUTE policies governing student behavior, including but not limited to, the Pantheon Institute COVID-19 Student Compact.
RESPONSIBILITIES OF STUDENT. As a participant in this PROGRAM, I agree to: 2.1 Be pre-screened for fieldwork placement or employment, be fingerprinted and participate in the criminal clearance process through the COUNTY or placement agency. If I have ever been convicted of a felony or misdemeanor crime that shall disqualify me from employment in public mental health, I shall be ineligible for, or terminated from, stipend eligibility. I shall then be responsible for the repayment of any and all money awarded for stipend as well as collection costs. 2.2 Satisfactorily complete the courses and field placements designed by UNIVERSITY to meet the requirements of the MSW PROGRAM, maintaining good standing in the UNIVERSITY and in the Social Work Program. 2.3 Unless specifically waived by the COUNTY Stipend Committee, arrange for all needed transportation as necessary for field placement and employment and possess a valid driver’s license and auto liability insurance. 2.4 Render twelve (12) months of continuous and satisfactory full-time employment beginning in a qualified entry level mental health position for each academic year of stipend/graduate study. I commit to return to, or accept, employment in the COUNTY or a contract agency that provides public mental health services to the county that provides my educational stipend. I may be assigned to any other regional office in the order of priority established by the agency director. Appointment to and continuation in all such positions shall be subject to all applicable county civil service rules and policies. 2.5 Report to work at the assigned position or apply to work at an open position in COUNTY or contract agency that provides public mental health services to the county within six months of graduation (see UNIVERSITY/COUNTY Stipend Agreement, Items 9.1, 11.1 and 11.2) from UNIVERSITY with a Master’s Degree. 2.6 Should no qualified mental health position be available with the COUNTY or contract agency of COUNTY that provides public mental health services, within six (6) months of my graduation, I may request permission from the COUNTY Stipend Committee to accept a position in a related COUNTY program or non-profit agency serving COUNTY mental health clients, or another California county Department of Mental Health. 2.7 Should no qualified position in a contract agency of COUNTY that provides public mental health services or other California county department of mental health, or related county program serving county mental health clien...

RESPONSIBILITIES OF STUDENT a) Each student must complete a Dual Credit Application online and provide all documentation required for DSU enrollment. The application process includes: submission of official ACT/SAT scores from the testing agency (school codes are 2190 and 1163 respectively) or ACT Tape on high school transcripts, high school transcript, permission from the school principal or guidance counselor, and approval from the parent or legal guardian on a signature form. b) Students must have completed a minimum of fourteen (14) core high school units and have a minimum cumulative 3.00 grade point average on a 4.00 scale OR have a minimum cumulative 3.00 grade point average on a 4.00 scale and earned a minimum ACT composite score of 30 or the equivalent SAT score. A minimum ACT composite of 16 is required for admission to DSU. Minimum ACT scores must be met for some discipline specific courses. For example, a 17 or better sub-test score in English is required to take a dual credit course in English Composition; a 17 or better sub-test score in Reading is required to take a dual credit course in History or Literature; and a 20 or better sub-test score in Math is required to take a dual credit course in a college-level math course. c) A qualified dually enrolled high school student shall be allowed to earn an unlimited number of university credits if available at their high school. d) Acceptance to the dual credit program does not qualify the student for full-time freshman admittance. To be considered for admission to DSU, the student must apply for admission as a first-time freshman and meet the admission requirements as set forth by the IHL. e) Participation in the dual credit program does not affect eligibility for freshman-level scholarships.
RESPONSIBILITIES OF STUDENT. The above student agrees to report promptly and remain throughout the assigned period unless he/she has received a prior excuse from the teacher and/or requested leave from the training site.

PPD skin test and, if positive, a chest x-ray, if negative, symptomatic monitoring yearly by a physician. b. Rubella vaccination (MMR) or titer. c. Varicella vaccination, titer, or proof of disease documented. d. Hepatitis B series, titer or declination. e. COVID Vaccination or medical or religious exemption per County Public Health mandate.

RESPONSIBILITIES OF STUDENT a. STUDENT shall coordinate with ▇▇▇▇▇▇ PERMANENTE’S Volunteer Service Department (“Volunteer Services”) for health clearance requirements, which include but are not limited to: tuberculosis clearance, Rubella (German Measles), Rubeola (Measles), and Varicella (Chicken Pox). All required information must be provided to ▇▇▇▇▇▇ PERMANENTE prior to STUDENT’s arrival at the ▇▇▇▇▇▇ PERMANENTE facility. b. If required, STUDENT shall provide his or her learning objectives and its evaluation forms and guidelines for the service learning program to ▇▇▇▇▇▇ PERMANENTE. c. Volunteer Services shall ensure that STUDENT completes ▇▇▇▇▇▇ PERMANENTE’s orientation requirements, to include signing all required ▇▇▇▇▇▇ PERMANENTE orientation forms and completing all applicable compliance training, prior to participating in the program at ▇▇▇▇▇▇ PERMANENTE. d. STUDENT shall comply with applicable policies of ▇▇▇▇▇▇ PERMANENTE as may be in effect from time to time; observe and conform to all laws, rules and regulations, customs and ethics relating to School’s operations; and comply with all state and federal statutes, rules, regulations and policies, to include but not be limited to compliance with anti-kickback statute,
