Supplemental Data. The RECIPIENT(s) understand and agree that any supplemental DATA obtained through the NCTN/NCORP Data Archive, The Cancer Imaging Archive, or directly from the Network Group are subject to the terms and conditions contained within this agreement. By submission of this DUA, the RECIPIENT(S) attest to the APPROVED USERS’ qualifications for access to and use of the DATA and certify their agreement to the NCI principles, policies, and procedures for the use of the DATA as articulated in this document.

Borrower shall submit to Lender a statement of the projected cost of constructing the Improvements, including a description of all contracts let or to be let by Borrower for the design, engineering, construction and equipping of the Improvements, setting forth the name or names of the contractor or contractors, the date of the contracts and of any supplements or amendments thereto, the scope of the work covered thereby, and the aggregate amounts payable to the contractors thereunder, and further stating whether said contract or contracts embrace all of the work required to be done and all of the material necessary for completion of construction, and, if not, setting forth sufficient information to enable Lender to determine the estimated cost of any work or materials not so covered.
Supplemental Data. AirPlus agrees to make every effort to provide Customer with the supplemental data it has requested through both the billing statement and via the AirPlus Information Manager (if used). However, Customer shall not be entitled to withhold payment from AirPlus on the grounds that any supplementary data does not appear on Customer’s statement or appears to be incorrect.

Without limitation of Gilead’s rights under Section 2.3.5 or ARTICLE 5, at any time during the Research Term for a Research Program, the Parties may, through the JRC in accordance with Section 2.3.2, modify the applicable Research Plan to include activities to be conducted by Gilead to generate additional pre-clinical data for such Research Program (the “Supplemental Data”). The availability and finalization of such Supplemental Data shall not in any way (a) excuse any delay in the delivery of the Preclinical Data Package by MacroGenics to Gilead in accordance with Section 3.3.1 or (b) affect the termination of the applicable Licensed Program under Section 3.3.2(c) in the event Gilead fails to provide notice of its intent to pay the applicable Preclinical Milestone in accordance with Section 3.3.2(a) or extend the Evaluation Period in accordance with Section 3.3.2(b).

(A) One (1) clear copy of an unexpired government-issued identification evidencing nationality, residence and bearing a photograph, signature, nationality, date and place of birth (e.g., identification page of your passport or U.S. driver’s license if a U.S. resident).
(B) One (1) current certified copy or original of a Bank or Legal Reference Letter or Utility Bill issued within the last three (3) months that confirms the subscriber’s residential address. Note that we cannot accept mobile telephone bills or bank/credit card statements. If the subscriber is not a natural person, please furnish the following supplemental data:
(A) One (1) copy of the formation document or other documentation evidencing the existence of the subscribing entity (e.g., certificate of formation, certificate of limited partnership, certificate of incorporation, partnership agreement or trust agreement).
(B) One (1) copy of the memorandum and articles of association, articles of incorporation, operating agreement, trust deed or other equivalent governing document of the subscribing entity.
(C) Document identifying authorized signatories.
(D) One (1) copy of the latest audited financial statements of the subscribing entity (where available).
