Annual Performance Reports. The Business must submit an Interim Performance Report one year after the Execution Date and a Final Performance Report two years after the Execution Date, both in the form of the affidavit attached as Exhibit III.
Annual Performance Reports. The Organizer shall publish an annual performance report that provides the information required under IC 20-20-8 in the same manner that a school corporation publishes an annual report under IC 20-20-8 and, if the Organizer or the Charter School maintain a web site for the Charter School, both that report and the section of the Accountability Report prepared by OCS and relating to the Charter School shall be made available in a reasonably conspicuous manner on the Charter’s School’s web site. The Organizer shall notify the parents of students of the Charter School of the availability of the reports using the same method of communication used by the Charter School for other regular communications with the parents.
Annual Performance Reports. Provider agrees to submit Annual Performance Reports, as requested and in the format required by DHS, within ninety (90) calendar days following the end of the State fiscal year.

Annual Performance Reports. Enterprise Technology Services will develop and deliver a draft annual performance report, analyzing actual performance results achieved and corrective actions implemented during the previous year for each measure identified in the SLA document. Based on the information shown on the draft annual performance report, members of the CUB governing the program/ Service Enterprise will have an option to review and provide feedback on any corrective actions recommended by Enterprise Technology Services to address non-compliance with performance targets, as well as to monitor the implementation of the action plans agreed upon with Enterprise Technology Services throughout the previous twelve months for the measures that did not show compliance with the established performance targets. The annual performance report will include:
1. The same information as the quarterly performance reports for the performance of each measure identified in the SLA document over the last quarter of the year.
2. An additional section with follow up information about the corrective actions implemented and the results achieved for the measures where performance was not in compliance with the agreed SLE’s in any given quarter within the year. CUB members will use this Annual Report to conduct a yearly performance review, which will be deeper and broader than the regular performance reviews conducted every quarter between the CUB and Enterprise Technology Services. The recommendations and feedback provided by the CUB will be incorporated by Enterprise Technology Services into a final version of the annual performance report. The annual performance review process could lead to a review and/or amendment of the SLA document agreed between Enterprise Technology Services and its customers. The final report will be posted electronically on Enterprise Technology Services website.

Annual Performance Reports. Yearly reports discussing overall grant performance and activities will be prepared at the end of the fiscal/grant year and submitted to EPA. The Annual Evaluation Report summarizes all data and the degree that outputs and outcomes are achieved.

The Recipient shall submit semi-annual performance reports into the GrantSolutions system for review and acceptance by DHS as a condition for receiving further annual funding increments. a. Performance reports must provide information on the overall progress of the Ce nter based on the activities discussed in the corresponding work plan. These reports should map work plan activites (activities planned) to those activities performed during the year to include:
i. Summary reports on the Center’s strategic vision and support justification
ii. Summary of Center communication and transition activities;
iii. Summary of Center management efforts including decision making apparatus;
iv. Performance reports on each Center Project should include: o Explanation of any changes from the initially approved workplan o Objective/Purpose o Baseline o Methodology o Project milestones o Performance metrics used to evaluate progress & assessments of current concept of operations and baselines/state of the art in use o Transition plans to include development steps, intellectual property management plans, & market specific considerations o Stakeholder engagement o Potential programmatic risks to completion; and, o Progress against each milestone outcomes and outputs and explanation of why any items were not reached o Unanticipated problems and plans for addressing them; and o Information supported by data on how project outcomes will advance or impact current technologies or capabilities.
v. Budget information (narrative and figures) categorized by both object class and project as described in Article I, A, item 3.v.
vi. If applicable, include a certification that no patentable inventions were created during the budget period.
vii. Updates to the Center’s Information Protection Plan and Researcher Safety Plan as needed.

Annual Performance Reports. The Subrecipient shall be required to submit an annual “Performance Report” to the Director of Planning and Community Development, via email, at ▇▇▇▇▇▇▇▇@▇▇▇▇▇▇▇.▇▇▇, which includes key performance indicators for each project, service, or deliverable, as well as both output and outcome measures. The key performance indicators and outcome measures will be mutually agreed upon by the parties. Output measures, (for example the number of students enrolled in an early learning program), provide valuable information about the early implementation stages of the project. Outcome measures, (for example the percent of students reading on grade level), provide information about whether a project, service or deliverable is achieving its overall goals. The Subrecipient must include the related outcome goal for its project, service or deliverable and provide updated information on achieving these outcome goals in its annual Performance Report during the month of August each year.
Annual Performance Reports. In addition to the quarterly facility reports, recipients must provide annual progress reports due to the EPA Project Officer each October 31. As noted above, these reports must include an update on the Maintenance of Effort. Further, in accordance with 2 CFR 200.328 and as specified in P.L.114-322, recipients agree to comply with the requests for data related to the use of the funds under Section 1464(d) of the Safe Drinking Water Act (SDWA). In addition, the States and the District of Columbia agree to provide in its report information regarding key project characteristics, milestones, and environmental/public health protection results in the following areas: 1) achievement of the outputs and outcomes established in the Implementation Workplan; 2) the reasons for delays if established outputs or outcomes were not met; 3) any additional pertinent information on environmental/public health results pertaining to test for lead in drinking water in schools or child care facilities. These activities include the development and provision of training courses, roundtables, webinars, tools, other products, and outreach materials. For each of these activities, recipients must report on their outputs and outcomes such as the types of actions taken to reduce lead in drinking water in the tested facilities; the number of communities with increased information about the health effects of lead, the number of facilities implementing drinking water routine maintenance programs, the number of school and child care personnel that received training on lead in drinking water, and other outcomes that support protecting children from exposure to lead in drinking water.
