The Recipient must compile and submit all information for funded Recipient Programs or programs, including the proposed measurable outcomes and all other items required under Minn. Stat. § 3.303, subdivision 10, to the Met Council and, if requested by the Met Council, the Legislative Coordinating Commission as soon as practicable or by January 15 of the applicable fiscal year, whichever comes first.

Eighty percent (80%) of participating parents will report (on the quarterly parent survey) that they have learned and applied at least one parenting practice while participating in Contractor's PAT home visiting. 7.2. One hundred percent (100%) of families who report moderate to high-stress levels on the quarterly parent survey will be referred to the Strong Beginnings Social Worker for further evaluation. 7.5. Sixty percent (60%) of participating families will attend at least one (1) group connection gathering quarterly. A) Sixty percent (60%) will connect to the resources. 7.6. Eighty percent (80%) of participating families will be provided with at least one (1) resource connection quarterly. 7.7. Sixty percent (60%) will connect to the resources. 7.8. Ninety percent (90%) of clients referred will be contacted within five (5) business days. 7.9. Ninety percent (90%) of referred clients will have contact attempted to schedule an appointment to enroll them in the program within ten (10) business days.
