Institution Responsibilities. 1. The Institution will use its best efforts to select students for participation in the A.L.E. who are prepared for effective participation in the training phase of their overall education. The Institution will retain ultimate responsibility for the education of its students. 2. Prior to the commencement of the A.L.E., the Institution will, upon request and with proper authorization, provide responsible Facility officials with such student records as will adequately disclose the prior education and related experiences of prospective student participants. 3. The Institution will use its best efforts to see that the A.L.E.s at the Facility are conducted in such a manner as to enhance the resources available to the Facility for the providing of care to its clients and patients. Only those students who have satisfactorily completed the prerequisite courses of their curriculum will be selected for participation in an A.L.E., as specified in the curriculum course descriptions. 4. The Institution will not assign any faculty member to the Facility in connection with the operation of the A.L.E. who is not appropriately licensed or certified, and will make evidence of the licensure or certification of all its assigned faculty available to the Facility upon request. It is agreed that all Institution faculty are employees of the Institution, unless otherwise agreed upon in writing. 5. The Institution will inform all its participating students of the Facility's requirement that they must procure and maintain throughout the A.L.E. professional liability insurance in amounts, form, and by a carrier satisfactory to the Facility and the Institution, and covering their activities at the Facility, and to provide evidence of such insurance to the Facility prior to participation in any A.L.E. Institution faculty members will be provided professional liability coverage pursuant to the terms and conditions of the Georgia Tort Claims Act (O.C.G.A. 50‐21‐20 et seq.). The Institution will provide Worker's Compensation Insurance coverage for its participating faculty members. However, the Institution will not provide Worker's Compensation Insurance or other insurance coverage for its students. This paragraph will survive the termination of this MOU. 6. Institution Responsibilities. The specific responsibilities of the Institution are: 1 To plan, develop, implement and evaluate the Program at Facility, and to provide Facility with copies of current course outlines, course objectives, and curriculum philosophy.

Institution Responsibilities. 1. The Institution agrees to comply with applicable Federal WIC program regulations by: a. Allowing WIC clients access to their WIC foods without constraints. b. Ensuring that WIC foods do not become a part of the communal food supply. c. Allowing WIC foods to be available exclusively to the WIC client who the food was intended for. d. Providing all other foods the WIC client would normally receive as a resident of the institution in addition to the WIC foods. e. Ensuring that the client has access to WIC services including nutrition education and referrals. f. Certifying that the facility will not receive any financial gain from the WIC client’s participation in the WIC program. 2. The Institution will immediately notify ITCA when the facility ceases operation or changes ownership. 3. Changes in name or address will be sent to the ITCA within ten days of such change at the address below. 4. An authorized representative of the Institution will accept training from ITCA staff as necessary. 6. The Institution will not treat WIC clients differently from other clients. 7. The institution will notify ITCA immediately if it ceases to meet any of the conditions outlined in the agreement. 8.

The Institution’s Chief Executive Officer (“CEO”) is responsible for communicating, on a regular basis, the priorities and long-term plans of the Institution, as approved by the Board of Regents, to the Foundation. 1.3. The Institution shall maintain a website identifying all private support organizations that support the Institution. The Institution CEO and Institution leadership will work, in conjunction with the Foundation, to identify and pursue opportunities for the utilization of STAR Park and other real property holdings of the Institution through public-private partnerships and other commercial lease arrangements, and the administration of research agreements, that are appropriate for a Foundation in keeping with the Institution’s mission and values. These transactions shall be evidenced by real estate transactions to be entered into between the Institution and the Foundation and/or the Foundation’s subsidiaries or other corporate structures. 1.7. The Institution recognizes the critical role played by the Foundation in advancing the mission of the Institution and agrees to support and to work in collaboration with the Foundation through the recruitment of collaborative entities supporting research, development of physical assets through public-private-partnerships and grant opportunities and, subject to applicable law, financial support where necessary pursuant to the terms contained in this MOU. 1.8. Institution Responsibilities. Students may access the University of Texas Health Science Center at Houston ▇▇▇▇▇▇▇▇ Medical School Internship Program by Institution Program referral or assignment, in which a faculty member (such as a Field Practicum Coordinator or Professor) refers or assigns one or more students for placement in the Internship Program, one-on-one Internship or practicum, clinical rotation or other structured educational experience. When a student is accepted into an Institution or DSHS educational experience or internship to meet the requirements of the Institution Program’s academic curricula or degree program, the Institution Program will: (a) Designate a member of its faculty, as the Institution Representative, to coordinate with the designated DSHS Program Liaison; (b) Provide to the DSHS Program Liaison or lead staff or designee written notice of the name and contact information of the designated Institution Program Liaison for conducting and coordinating the Institution’s ▇▇▇▇▇▇▇▇ Medical School or other related educational experiences; (c) Require participating students and faculty to abide by the relevant laws, rules, regulations, policies and procedures, and best practices in relevant practice field(s), as designated by DSHS and the Institution during the term of the educational experience and while using the corresponding facilities and equipment, as specified in the Primary Agreement. This includes, but is not limited to, privacy guidelines and HIPAA, as specified in the Primary Agreement; (d) Require the students to be properly attired when reporting for the educational experience, as communicated by the DSHS Host Program and/or Educational Site; (e) Ensure its faculty sponsor or representative, if applicable, and student interns cooperate with and participate in the DSHS in-service programs or training, as required and directed by the DSHS Intern Supervisor; (f) Maintain control of its Academic Programs/Curricula; (g) Assume the responsibility for repair or replacement of equipment that is broken or damaged due to the negligence of a student or faculty member. (h) Remove a student from the DSHS Internship Program or educational experience when Institution determines that the student has violated DSHS rules, regulations, policies or procedures, or has engaged in conduct that disrupts the activities carried out by the Institution Internship Program, or threatens the safety of DSHS or Institution’s service recipients.

The Institution will respond constructively to any feedback. 9.4.3. The Institution will investigate any formal Research Degree Candidate Complaints in accordance with the University Complaints policy. 9.4.4. The Institution will investigate any formal Appeals in accordance with the procedures laid down in Regulation 36: Student Appeals. 9.4.5. The Institution will conduct regular surveys of Research Degree Candidate satisfaction, locally or as part of national surveys.
